The HEENT assessment and the Skin, Hair, and Nails assessment report based on Tina's condition will include medical history about the HEENT system, Tina family history regarding the HEENT, and the reviews of systems related to the HEENT. The report will also incorporate the medical examination and history of Tina’s hair, skin, and nails (Shadow Health, 2017). Other factors to be considered in the report include the symptoms of the target systems, including their characteristics, duration, and location. The alleviating factors, onset, and treatments tried will also be considered. The report will indicate common abnormalities found in the physical assessment of the HEEENT and also provide the pathophysiology of the problems established during the evaluation (Jarvis, 2015). The skin, hair, and nails assessment in the report will describe the anatomy and physiology of the integumentary system, and provide common skin lesions identified in the evaluation. 

The two SMART goals for Tina will include specific and measurable goals. The specific goal will be based on what is going on with the HEENT, hair, skin, and nails, why, and how it is going on. The specific goal will enable the assessment to cover specific aspects of the patient and how to accomplish the treatments required. The second goal, which is measurable, will provide the best way to manage Tina conditions. The measurable goal involves obtaining statistical facts regarding HEENT, skin, hair, nails, and compare their discretion with recommended or standard conditions to measure Tina’s health. Obtaining measurements in HEENT systems will enhance the ability to design the best intervention approach. The evidence-based practice nursing interventions that can be applied in the HEENT system, hair, skin, nails assessment include the nurse-patient communication-based interaction. The communication approach will offer an opportunity for the nurse to obtain information regarding the history of the condition, and other symptoms that might not be visible or assessed through laboratory tests. The second evidence-based practice to be applied by the nurse is investigating the pathophysiology of problems related to the HEENT system, hair, nails, and skin (Risch, 2019). The investigation approach requires the nurse to develop a PICOT question, which identifies the problem, comparison, intervention, outcome, and timing, and conduct literature research to achieve better assessment.
