Additional questions to help start a care plan 

In addition to what the symptoms the patient has presented with the combination of the results tests, additional questions and information are needed to start a care plan for the patient. Further questions and information will help to better understand the condition and current health status of the child. The first question to be asked is, how often does the patient instead the boy drinks milk? This question is necessary to the doctor as milk can do well to the body as well as can be harmful to the body. Cardiovascular diseases, obesity, and type 2 diabetes are common health problems if too much milk than a considerable amount is taken into the body (Thorning et al., 2016). Therefore much information from this question can help determine the health of the patient. 

Secondly, the patient’s mother can be asked if the patient has a history of allergies, for instance, a hypersensitivity to perfumes. The doctor will have a better understanding of the nasal congestion of the patient if this question is asked. Thirdly, how often does the physical education session (P.E) take place, how long does it last and under what kind of weather does the P.E. often take place? The doctor will better understand Exercise-Induced Bronchoconstriction of the child patient if the question is asked.

Assessments/Conclusions to document when building a problem list 

The patient may be suffering from short, or long term health complications brought about by premature birth by the mother at 34 weeks (Kodjebacheva and Sabo, 2015). For this reason, the patient needs to undergo severe observations and in-depth laboratory tests from different specialists. A nutritionist can be involved to check the weight of the patient and his level of obesity as well as whether the consumption of milk should be reduced or not. In addition, pulmonologists or otolaryngologist can be involved to work on the respiratory complaints by the patient. 

For proper continuation of medical care of the patient, the history of the patient’s health is required in order to be revised. Information of the family health history can also be inquired to know whether there are any forms of inheritable health problems. It would also be necessary to understand whether the environment of the school, specifically of the class is suitable for the patient or if he finds it uncomfortable. This can help lead to a conclusion of why the patient is not completing and submitting his class assignments.
