One of the models used in community health assessment is the Association for Community Health Improvement, Community Health Assessment Toolkit. This model was initiated by the American Hospital Association and was last updated in 2011. It is a toolkit used for planning, leading as well as using community health needs assessments (CDC, 2019). This model is well known for providing a six-step assessment framework which is accompanied by practical guidance. One will require a paid membership in order for them to access the full toolkit.

What are some benefits of using a specific model to assess a community? 

There are various benefits associated with a community health assessment. One of the advantages includes the increased understanding particularly about the needs of the community as well as the reasons as to why they exist, and why it is important for their needs to be addressed. The strengths and weaknesses of the community are also identified and addressed. Another benefit is that the community members have the chance of sharing how the needs affect the quality of their living even for the whole community (CDC, 2019). 

What agencies should be included with the assessment? 

Key agencies that should be included in the community health assessment include agencies like CDC which stands for Centers for Disease Control and Prevention, and then there is also the National Institutes of Health (NIH) and finally the Agency for Healthcare Research and Quality (AHRQ). These agencies will make sure that the process is successful and quality data is achieved without violating the rights of the community members involved (Koh, Rajkumar, & McDonough, 2016 p1039). 

What is a reasonable time frame in which to complete a community-wide assessment? 

The required time in order to complete a community-wide assessment is three to five years. Throughout this period, the assessment should be completed and updated in order to make sure that the newly emerging issues and needs are always identified and addressed accordingly. This will also make sure that the health agencies involved covers every area that is targeted without living any member in the community out (Koh, Rajkumar, & McDonough, 2016 p1039). 

What is the role of the public nurse in the implementation of the chosen model? 

The nurses play a huge role in the implementation of this assessment model. They ensure that the assessment process is successful through working within the assessed community and making sure that they are delivering preventive healthcare services (Choi, 2015 p52). They also provide health-based information by instructing community members in the care and rehabilitation of patients as well as teaching home nursing at the community level.
