Because of the rising cases of individuals suffering from acute episodes of asthma, it is necessary to shift the focus of healthcare systems from just treating the already diagnosed patients to managing the problem since prevention is better than cure. In order to achieve this, a plan for providing a program for asthma disease management must be developed. The goals and objectives of this plan will be to: save costs associated with asthma treatment, improve the quality of healthcare to asthma patients, early identification of undiagnosed patients and teaching patients and their families’ self-management skills.
In selecting the program’s participants, the hospital administration will identify participants by developing a stratification methodology that will rely on health assessments, risk stratification, claims data and predictive modelling ( Arora et al, 2008 ). However, the program already has a target population with a specific disease which in this case is asthma.

The components of asthma disease management plan include: prevention, Pharmacologic Management, patient tracking and follow-up. The program aims at providing education to asthma patients and high risk members on optimal inhaler techniques where clinicians will evaluate the patient’s inhaler technique regardless of whether the patient has been using the inhaler for a long period. The high risk members will be taught on how to manage asthma through self-management skills which involve avoiding asthma triggers such as second-hand smoke and pet dandruff (environmental control) ( Kallstrom & Myers, 2008 ).
In order to ensure that the program is beneficial and effective, the program’s effectiveness will be evaluated using a three step process namely: the programs process (whether the programs policies and interventions have been implemented successfully); the programs intervention impact and the programs health outcomes ( Arora et al, 2008 ). This measures will assist in measuring whether the program has achieved the above stated goals and objectives.
