The primary purpose of collecting health history is to help in deciding on treatment and diagnosis (Heron 2018). Besides, it helps in determining what people expect from the patient and the health practitioner. In this case, the 14year old may not be in place to answer all the questions necessary for this case, so the guardian who is the grandmother will help in providing some of the information. The physicians will also gather data from previous records. Communication technique to be used in this case is vital because it helps in building an environment where the responded will be willing to share the information. One of the techniques that I will apply is sticking to the open-ended question, which allows the patient to provide more detailed information. Again, since this is a 14year old boy, I will assure him of confidentiality and privacy since individual at this stage requires own space. 

According to Joseph et al., (2019), avoidance of medical jargon and acronyms will help in better understanding and confusion will be limited. I selected the family history tool as the risk assessment tool that will help in evaluating the patient medical history (De Massis et al., 2019). The reason why I choose this tool is that it provides an outlook on the medical condition related to inherited genes. Besides, most of the diseases that affect children at his age are hereditary. Questions the interview will include asking the patient about their medical experiences. I will also ask the patient about any conditions physicians diagnosed him recently. The grandmother, who I assume is the guardian of the 14-year-old, will also provide information about the current medical history and their health maintenance habit )Bajracharya et al., 2019). For instance, the guardian will elaborate on the environmental factors in their residential place that may affect them as they expose people to toxins or chemicals that may trigger any illness.
