Case management includes a highly collaborative process where there is assessment, facilitation, coordination and evaluation of care with advocacy for the right mix of options and services to provide to a patient that meets their own and family’s comprehensive needs by determination of available resources that that would lead to optimum patient safety, quality of care and cost effectiveness of outcomes. Case management is based on the premise that everyone benefits where an individual reaches the optimum level of health. Advocacy, education, communication and identification of resources to facilitate the attainment of the wellness of a patient is the real purpose of case management. Based on this background, the case study is in line with the basic principles of case management studied in class. 

In responding to the health needs of the young lady, the case managers not only assess her physical well-being but also make consideration for her mental and social wellness. Generally, the young lady presents with seizures. However, the case managers considers her mental health including depression and her low IQ that would affect her ability to take medication. The IQ of the patient is seen as a compromise to increased seizure. Further, the case managers consider the factors related to her mental health issues. Through the assessment, they discover the lady lost her mother and was left in the hands of a father who was not as caring. Further, results indicate that the lady wanted to have a boyfriend and live a normal live like her friends. Notably, the father’s conditions made it difficult for him to effectively take care of the lady.

Moreover, it fits in the on-going consideration of family centered care as studied in class that involves considering the social and economic conditions among other factors. Notably, the case managers in this study consider the economic position of the family including the father’s ability to pay medical bills. For example, when depression was not covered in her father’s medical plan, the case manager acquired the approval from the administrator to get full reimbursement. Also, a proper understanding of the family’s past including the death of the lady’s mother allowed for proper consideration of her care plan. The ability to assess and utilize resources available to the family is also displayed by the case management team in this case study. For example, in the case where the girl could not partake in counselling due to funds, the case manager identified a therapist near and negotiated the fees. 

Question 2 

As Dr. Daniel Kraft says, there is a need for the continuous evolution of case management to catch up with the increasing dynamism of the health care environment. The field involves interacting with different issues affecting healthcare and the society in general including technology. Being the point of unification of medical services provision to individual patients, the role of case manager must evolve accordingly to accommodate the changes if it is to remain relevant to the health system. For example, as technology continues to change the healthcare sector, it impacts case manager. The case manager must remain well informed of existing and emerging new technologies including their benefits to individual patients in order to identify the right mix of services that will bring optimum health to their clients. 

While technological changes continue to impact the way in which healthcare is delivered, case managers must observe the ethical principles drawn in the Code of Professional conduct for Case Managers. This code acts as the guiding principle for practice in the dynamic healthcare sector. Consequently, any technological advance adopted by case managers must be in tandem with the code. This includes ensuring the protection of public interests and the well-being of a particular patient during assessment, planning coordination, facilitation, and evaluation of advocacy services.
