Case management in nursing is a systemic, collaborative, and dynamic approach that aims at providing and coordinating healthcare services to a specific population. There are components of case-management, intake, needs assessment, service planning, advocacy evaluation, and monitoring. Intake involves the gathering of demographic information about a population, the identification of need needs, and then build a relationship with the population. Needs assessment collects more information on the population’s specific problems, the risk to success, interests, and challenges. During service planning, the nurse develops goals and methods of meeting the goals (Keller et al., 2004). Monitoring and evaluation are aimed at understanding the impact the program has on the population and also measure its success and also evaluating the strengths and weaknesses of the program. 

Services planning should detail the anticipated needs of the population and provide a framework for achieving the desired goals. The target population is the substance abuse group which requires specialized treatment. During planning, the specific processes of providing services to the group should be developed. The plan should have strategies aimed at the substance abuse group. The role of a public health nurse is to track the patients’ progress towards the achievement of the goals. The public health nurse help in supporting the patient’s self-management so that their health status can improve. The PHS is responsible for assessing and evaluating trends and risk factors of the group and develop a plan that has priorities for the targeted interventions (Pranboon et al., 2020). The PHN should also work with the specific group within the population and help in the development of public policy. He/she should also provide health education to the substance abuse group and introduce programs that will create awareness for the entire population on drug and substance abuse. The PHN should plan interventions that would address emerging patterns that would otherwise compromise the efforts put in place to address substance abuse.
