Rosalina Sanchez is a student nurse on her community health rotation. She is working with a registered nurse providing care in a rural health clinic. The clinic serves a large population of migrant workers and young families. The majority of the clientele speaks Spanish, which is Rosalina’s primary language. During her time in the rural health clinic, Rosalina learns that access to health care is a concern due to cost and transportation. Another issue among the migrant population is continuity because workers tend to move according to seasonal farm work. 

The public health nursing care that Rosalina would expect to experience in the rural health setting includes nursing care which is culturally diverse and appropriate to the needs of the community. The cultural diversity aspect is important since workers here move as dictated by seasonal farm work. Also, Rosalina would expect to experience a treatment aimed at preventing illness and promoting wellness to lessen access to health care issues as a result of cost and transportation. Further, she would experience the illness care provided to the individual with referrals to acute care facilities in metropolitan locations. Since this is the only primary health clinic serving a large population in the rural, resources are overstretched, and there are inadequate facilities to serve the population.

The type of primary prevention activity Rosalina would expect to encounter in a rural health clinic is a health education program for school-age children that teach them about the health effects of smoking. Since the migrant population are engaged in seasonal farm work, it is anticipated that they are involved in tobacco farming. Rosalina would as well expect to encounter in the rural health care toxin screenings for migrant workers who may be exposed to pesticides. Pesticides could directly be inhaled by farmers who spray the crops during farming. Nonetheless, she would expect to encounter an advocate for the underserved and disadvantaged population. Since the community in the rural area is composed of a majority of the migrant population of Spanish origin, there must be a person exploiting them as the underprivileged and disadvantaged population. 

One of the core public health functions is an assessment of the health status of a community. The appropriate questions that would be asked in the assessment phase are: 

What programs are available to reduce the risk of cardiac disease in the population? The question is important since smoking tobacco induces cardiovascular disease. 

What educational materials are needed to inform the community about environmental risk factors? This question is intended to highlight the biomarkers to identify tobacco smoking-induced cardiovascular disease and therapy implications of the disease’s pathophysiology. 

What is the prevalence rate of hypertension among various age, race, and gender groups in the community? The question focuses on identifying hypertension as the highest risk factor for cardiovascular among different races, gender, and age.
