Wound care is a term used to refer to the kind of services provided for the treatment of damaged skin. Wound care, therefore, provided with the intention of reducing damages that would likely affect the underlying body structures such as bones, nerves, tendons and veins. Wounds present a number of challenges due to its possibility to lead to more complications such as tetanus, long-term illnesses, which in turn may cause long-term disabilities ( Järbrink, et al., 2016) . Such might be caused due to the patient presenting their wounds late, increased number of casualties that overwhelm the provision of better health services. In case of such issues occurring, it would be better to prevent further infection, never close infected wounds, and not closing contaminated wounds. Under nursing care, all activities carried out in the treatment of a patient are recorded. The information recorded would be used for continuous treatment, quality care and compliance. 

Change Model Overview 

Academic Center for Evidence ‐ Based Practice Star Model is aimed at establishing an elaborate way to understand the various form of knowledge used in the evidence-based process. Nurses should use this model to provide a change in their ways of work because it offers a deep analysis of information gathered, it provides options on what would be undertaken in the study. In essence, it contains a framework through which evidence-based processes would be put into action. As such, nurses would have found a significant basis for undertaking their activities consistent with other legal frameworks in place ( Motley, et al., 2018) . This model is the most appropriate that nurses would be able to put their scientific knowledge into practice. In such ways, there would be an elaborate system which is easy to follow hence presenting an elaborate framework for undertaking actions.

Define the Scope of the EBP 

Wound care is a challenge to many that has caused unimaginable pain and use of resources after it led to more complications that result in long-term illnesses. The topic of wound care is a problem for some people who eventually end up having disabilities that were not expected in the first place ( Järbrink, et al., 2016) . In this regard, it is a critical topic for discussion since more people do not take it seriously when symptoms of skin emerge and would only act where the issues have advanced to critical stages. Studies show that 1-2% of the populations suffer from chronic wounds. In the US it is estimated that 6.5 million patients with chronic wounds are affected and the government spends $25 billion each year. In UK chronic wounds are estimated to cost the country about $4.5 billion in a year. This information is critical to understand that such issues are a concern towards sustenance of wound care. 

Stakeholders 

Project stakeholders would be nurses, pharmacists, therapists, wound care team and nurse aids working within the health station. 

Determine Responsibility of Team Members 

The chosen members are important since they would facilitate proper delivery of required services. For instance, the wound care team would be in charge of taking necessary actions while treating wound care patients. Therapists would be ensuring that the patients are taken through necessary healing transformation such as exercising. Nurses will administer the prescribed medication for the patient. Nurse aids would thus support the nurses in the discharge of their duties. 

Evidence 

There is evidence relating the effects of wound care how they are treated such as wound cleanings, pain relieving, dressing choices, and organizational aspects ( Ubbink, Brölmann, Go, & Vermeulen, 2015) . The evidence found to indicate the methods used to undertake treatment for the wounds. In relation to the EBP, most nurses undertake emergency care services hence overlooking the information that supposedly should be based on offering their services to the patients. In line with the need to support the provision of quality data, there are a number of challenges that prevent the provision of quality services. Nurses are required to provide relevant information based on EBP ( Motley, et al., 2018) . Also, the clinical practice guidelines are the basic measures that support service delivery for the nurses. This research offers an information on the critical areas that have to be improved to attain higher service delivery to patients. 

Summarize the Evidence 

The provision of better care services has to be provided based on known studies. EBP used in most clinical procedures should be applied by nurses at all times to provide high-quality services ( Ubbink, Brölmann, Go, P. & Vermeulen, 2015) . It was noted that evidence-based information offers to understand service delivery to the people. Such information would further enable the undertaking of information allowing future improvements on the field. 

Develop Recommendations for Change Based on Evidence 

Studies show that in most cases nurses overlook the EBP to provide services to their patients. It should be recommended that such negligent actions be stopped and consideration for the practices undertaken by nurses. Nurses would not overlook critical provisions to guide them at all times. This is important that it would allow for the consistent discharge of their duties.

Action Plan 

The action plan would involve first setting a goal. The team would set the goal that would be focused on and enable members to come up with appropriate mechanisms to attain the set goal. The second step would be to develop the process that would be followed in carrying out the activities. Such a process would communicate measures taken as well as providing review mechanisms on the way ( Motley, et al., 2018) . The final stage would be the implementation of outcomes. This stage takes into consideration aspects that would emerge while undertaking the implementation processes. The study implementation can take considerable time based on workload for the stakeholders. Outcomes of the study would be evaluated based on existing plans. 

Process, Outcomes Evaluation and Reporting 

Identification of the issues affecting wound care is the important aspects required to facilitate making improvements. Recording information would serve as a suitable way to be measured. Therefore it would support tabulating information that would then be handed to the stakeholders. 

Identify Next Steps 

Implementation will take into consideration important factors such as resources. Given that other sectors operate better, then it would be needless to effect an implementation process on them. To ensure that the implementations remain unchanged, it would be suitable to inform on its importance. 

Disseminate Findings 

Internal communication of such results would be made by direct communication to the stakeholders when having a meeting. While to communicate to the external parties, it would be made through publications. 

Conclusion 

Wound care is a critical issue in the health sector. Nurses handling it have to consider EBP as its basis. Studies have shown that the issue has continued as a result of negligence. Without proper care, wounds are said to result in disabilities among its victims. Academic Center for Evidence ‐ Based Practice Star Model has been noted to facilitate the elaborate process for undertaking changes in an institution.
