Self-management of chronic disease refers to measures undertaken by an individual to manage their health conditions without necessarily seeking medical attention or before seeking medical care. To ensure that individuals with chronic diseases are aware of the measures they can take to manage their conditions, programs that educate on self-management need to be implemented to assist in disseminating the necessary information (Martz, 2018). The programs are tailored to help individuals learn and practice various techniques that are required to build the confidence and individualized self-management to enable individuals with chronic diseases to carry it out.

Communicate clearly

Studies by Martz (2018) indicate that patients can comfortably understand complex health information when efforts are put in place geared towards health literacy. To effectively disseminate health literacy, a nurse practitioner requires communicating clearly. Health literacy takes place when a patient and a healthcare provider are in real understanding. Keeping a concise and straightforward language helps patients to relate effectively to health issues.

When information is disseminated, there arises a need to confirm knowledge and understanding. Establishing understanding is pivotal in chronic disease self-management since communication without understanding is of no use. In efforts to demystify complex health issues, drawings, pictures, and illustrations are an excellent channel to help patients understand health issues better. Common health scenarios, medication management, and surgical operations can better be explained by using models and visuals to assist patients to understand more what is happening in their bodies.

Healthcare providers can confirm and validate full understanding through making clarifications and questioning patients. To achieve such objectives, healthcare providers can create encouraging question-asking as well as employ open-ended questions to generate dialogue and personalize information (Carrier, 2016).
