Vila health care project required the collaboration of practitioners involved in caring for patients with chronic diseases to reduce the rate of readmission that the facility had been experiencing for the last six months. The reason for the need for collaboration was because of the myriad of health challenges shown by the patients. Several professionals are listed among this that need coordination and addressing. Among the professionals listed included doctors, nurses, occupational therapists, nutritionists, pharmacists, physical therapists, and social workers. 

As a physical therapists, I worked together with social worker to help a patient who had drug and substance abuse problem. As the social worker helped the patient to develop mentally, I helped him with enhancing his physical wellbeing, including healing the injuries he got from engaging in a fight with others. Reflecting on my past experience, I have learned that collaboration makes work easier. It helps with the subsequent healing of the patient. Teamwork enhances success, thus encouraging me to develop better relationships with other professionals. Developing good communication skills is the key to developing collaboration between practitioners.

Strategies for Managing Human and Financial Resources 

Human resources are responsible for designing the essential strategy for addressing the challenges the organization was facing. Among the strategies used include providing education to prevent the recurrence of the disease, maintaining environmental hygiene, and utilizing social media to collect feedback on the effectiveness of the services (Cogen et al., 2016). The best lesson from the resources concerns the effects of the lack of collaboration. According to Sullivan et al., 2015, poor collaboration causes underutilization of resources. Lack of teamwork hinders effective resource planning and utilization. Working together facilitates the maximization of support through the collaboration of skills and competence to achieve set goals. Poor collaboration also increases the time needed to perform a particular task individually. This increases the financial costs while at the same time, making it challenging to manage the workforce as opposed to when they are working as a team. For excellent collaboration, it facilitates effective communication and teamwork that promote the success of the team. 

Interdisciplinary Collaboration 

In a multidisciplinary collaboration, equal opportunities are essential for team members. In the case of the Vila facility, each practitioner was involved in brainstorming the ideas and generating the necessary knowledge (Pajalich, 2019). Effective environmental and social communication was crucial to collaboration. However, the team failed to achieve their goal because they could not consult from outside to help them make all-inclusive solutions (Rudolph & Zacher, 2020). The conflict on those that should be part of the team also disintegrated their connection. 

Role of Change Management Theories and Leadership Strategies 

Among the factors that affected the success of the project include leadership strategies and suitable change management theories. A democratic leadership style that encourages positive communication and participation, in general, is appropriate for the project. It gives team members to participate in decision making (Saunders et al., 2019). The application of Kotter's change theory could help ensure the success of the change. The approach provides not only the eight stages of the process but also suggestions on how best to deal with issues like resistance as well as eradicating ant form of barriers in advance (Pollack & Pollack, 2015). Understanding the stages of implementing change increases the chances of success in attaining the set goals. 

A nurse can use the theory to create a sense of urgency and eventually convince leaders and stakeholders on the need for the change. The next steps involve the formation of a strategic vision, getting everyone on board, removing barriers to reduce friction, and then generate short-term wins ( Pollack & Pollack, 2015) . The team must also seek measures to sustain the acceleration before anchoring it to the culture of the organization and the community at large. Ensuring constant and open communication is an effective strategy to ensure teamwork.
