Community Health Nursing II is a significant course for my professional development as it captures essential aspects of community health programs and initiatives. The course focuses on epidemiology, disease transmission, assessment of risk factors, environmental health, and collaboration with interdisciplinary teams to promote health outcomes. Community Health Nursing II is based on various course objectives, which guide the learner to understand community health care processes and standards. As a community health enthusiast, I have worked so hard to ensure that I am aligned with the course objectives to provide the best services to patients. The first objective that I have managed to achieve is enhancing my information technology skills so that I can be in a position to gather and analyze issues to do with community health. I understand that information technology has changed the collection, analysis, storage, and dissemination of community health information in enhancing decision-making processes (Kruse and Beane, 2018). I have managed to collaborate with various community health partners including financial, healthcare, and educational institutions, in offering healthcare education to improve healthcare outcomes.
Community Health Nursing II emphasizes the need to develop educational projects aimed at promoting the community’s health outcomes. So far, I have managed to create charts, brochures as well as lead support groups on type 2 diabetes to help individuals and families to report better outcomes. Over the last few days, I have been involved in the creation and assessment of various teaching projects to ensure that they are aligned with the health needs of the target community. According to Gruber et al. (2019), community health needs assessment provides hospitals with a roadmap on the most pressing health needs for redress. Having assessed teaching projects, I have gone ahead to implement them to ensure that the community health objectives therein are realized. The other day, I was involved in the circulation of brochures to young people who were addicted to drugs so that they could seek counseling services. From time to time, I present recorded evidence to demonstrate the achieved course milestones as they relate to teaching projects. This task allows me to know if I am aligned with the course objectives, which in turn demonstrates my readiness to improve the health outcomes of communities.
