On the first day at a home health agency known as the Doctors Choice Home health agency in Los Angeles, California a palliative care program is provided that guides care and supports their patients. In the upcoming week, my specific learning goals and objectives involved delivering timely, effective, equitable, and efficient health care to all members of society. Therefore, in order to achieve the goals and objectives of the home health agency, the SMART objective is the most important method of measuring healthcare progress. The SMART objective is specific, measurable, achievable, relevant, and time-bound. Therefore, professional, compassionate, and individualized healthcare is provided to patients with terminal and chronic diseases ( Skilled Nursing Home Healthcare Services LA | Doctors Choice , n.d.). The home health agency provides these objectives and goals so that the patients may live longer, inspire good health, and have an entirely comfortable home. 

At the community service placement, my upcoming weeks' complete scheduled practice experience includes proper care for patients in dire need of health care. Every staff member has the obligation of being aware of the healthcare challenges in providing necessary assistance. The other practice mandated is the provided communication channel, which can provide documented reports to other caregivers ( Baglin & Rugg, 2010) . Additionally, the high level of communication can keep the family members of the patients informed and updated on effective health care.

One of the placement issues that occurred this week in the home health agency I feel should be made aware to the instructor and private in nature is the changes in the environment, leading to an imbalance. Based on my understanding, the system in place can be interrupted by environmental changes such as family members, individual patients, community, and social gatherings ( Skilled Nursing Home Healthcare Services LA | Doctors Choice , n.d.). Human beings are bio-psychosocial in nature, and the changes in the environment can lead to physical problems that require the nurses in the home health agency to adopt methods of facilitating body and environmental changes. 

Therefore, the objective that I worked on this week involved the management of diabetic patients. Through the home health agency, I coordinated with other nurses, personal physicians, community, and family of the patients by creating a personalized treatment plan for the patient (Baglin & Rugg, 2010). The patient is provided with a supportive, comprehensive, and independent personal plan of treatment that will help the patient's well-being and health. The Doctors Choice Home health agency provides in-home medical care services that help manage the patient’s needs.
