There is a cover up and lots of lies in the medical field when it comes to medical errors occurrences. A report on National Canadian posts reveals a situation where a young lady by the name Monique Chisholm believes that her son’s disability was as a result of medical mishaps. Before birth, a fetal heart monitor revealed that Callan was suffering from hypoxia a disease that arises as a result of oxygen shortage in the blood stream (Blackwell, 2015) . Even After diagnostic results, the nurses induced the mother with Pitocin during delivery, and the drug meant to induce labor led to unfavorable income. 

Callan ended up living with a severe form of cerebral palsy, and today he can’t feed himself or walk by himself without support; this is unethical because according to the policy the operation/treatment should not cause any harm or result to a negative repercussion in the patient’s side. Ms. Chisholm reports that Cullan needs constant support, care, and supervision which has been far from easy to the mother (Blackwell, 2015) .

According to the Blackwell report, Chisholm’s situation could have been performed safely through C-section operation after noting that the fetus was in distress, this procedure could have prevented the critical consequence resulting from Pitocin induction. However, the fetal monitor system appeared to be broken because it revealed an abnormally slow heart rate just a few minutes before delivery, which forced the doctors to deliver the baby through Vacuum extraction so as to save the life of the child (Blackwell, 2015) . The system revealed that the umbilical cord had accidentally wrapped around the fetus neck, and later it became apparent that Callan had had a severe brain injury. 

This tiny fraction of medical error could have been avoided if only doctors had taken action and reported the issue. The strict rules, however, are designed with limited success in reporting internal mistakes. Also, the fear of being fired or blamed prevents most specialist from reporting incidences of medical mishaps. The rise of new technology with the new operating system is owing to the increase in error creation in many hospitals.
