The uniqueness position of the nurse practitioner makes them vulnerable to inevitable ethical dilemmas and tensions. Increasingly, nurses are interacting with patients with cultural backgrounds different from theirs. Furthermore, not only is the current healthcare delivery model patient-centric, but it is also value and evidence-based (Blair & Mirr, 2015). Thus, whenever a nurse practitioner encounters a patient whose values differ with those of their health promotion approach, it is usually a difficult experience for both and can erode the quality of care. In my opinion, it is prudent to respect and preserve the patient’s autonomy. Moreover, by appropriate communication, I can better understand the salient issues and uncover the patient’s general perception and expectations of the situation.

Discussion 2 

Health promotion among nursing practitioners involves balancing ethics and evidence-based systems, which does not always present easy solutions. The ethical issue I encountered was a young mother’s refusal to agree to tonsil removal surgery that would improve her underage daughter’s health and welfare. From my interaction with the mother, I realized that she was dependent on the advice and goodwill of her great aunt and that both had strong reservations based on their negative perception of surgery. The other ethical issue is the consideration of surgical necessity along with the impact of overruling the mother in a manner that was either threatening or arbitrary. The series of ethical situations result from the girl presenting another sore throat and her mother’s refusal of surgery.

The process I used to arrive at an ethical decision was the seven-step participative ethical decision-making model (Blair & Mirr, 2015). First, I established that there was a problem without a solution that completely satisfied those concerned. Finally, in order to find a solution that was universally accepted, I acknowledged their concerns and assuredly informed them of the advances in anesthetics and nursing.
