Health Risk Assessment (HRA) tools are used by health care organizations to monitor the health status of patients over a period of time. They are instruments used to monitor the health assessment of a patient and provide feedback on actions that can be implemented in order to maintain health and prevent disasters. In a risk management project, it is difficult to keep tracks on the kind of gargets, applications and different resources that one has already handled. A healthcare organization must have initiatives that manage uncertainty and risks that are likely to occur. Examples of such tools include fall risk assessment tool and Braden risk assessment tool. The tools stress the use of technology to manage risks in healthcare organizations. 

Falls risk assessment tool is a HRA safety initiative that prevents patients from falling. Nurses use the tool to evaluate the possibility of falling in patients who are elderly. Most accidents and injuries in healthcare facilities result from patients falling down. This is mostly seen in those above 65 years of age. The tool captures the patient’s history medical reports and equipment used. It then calculates an overall fall risk rate. The ratings help in prevention of possible falls for elderly patients.

Braden risk assessment tool accesses any possible problems that can occur in a patient developing pressure. It identifies the patients who are at high risk of developing a pressure ulcer. It is composed of six subscales that measure risk elements which either increase the intensity and duration of pressure or reduce the tissue tolerance for pressure. This involves sensory perception, activity, friction, shear and moisture. All the items indicate scores between 1 and 4 and all the scores are accompanied by a descriptor. When the score becomes lower, it means that the risk level is high.
