Community Practice Experience Placement Schedule
Make initial home visits to the patient (to do medication reviews, undertake adherence assessments, make further assessments of life risk factors, and give instructions on the home BP devices)
Doing interactive counseling sessions with the program coordinator: on topics such as BP and health, setting goals, managing medications, living with chronic conditions, diets and physical activities and effective stress management (Harrison, 2017).
Track the Medication management of the current patient in the community
Follow-up on individual patient’ planned diets and physical activity
Ensure proper stress management course is maintained.
Goal setting for the patient assigned.
Working with patient on medication adjustments (Palatnik, 2020).
Liaising with the primary care providers where necessary (by reporting on patient participation in the program, providing recent BP records, reviewing reported adherence, and reviewing medication management).

Addressing the challenges of getting the patient to adhere to the diet or physical activity programs.
Developing the best plan for keeping the patient’s pressure in check and helping the patient adhere to the therapeutic regimen.
Description of the week’s objective
The week’s primary goals revolve around examining the effects interventions that are in place for the patient. A comprehensive and objective analysis of integrating essential and augmented hypertension interventions in terms of outcomes, medications, and progress of the patient’s self-management (Keiffer et al., 2020). Priority will also be on estimating costs of respective intervention strategies, subsequent care, and compare the costs with other feasible strategies. Considering the patient could have different limitations, such as finances and caregivers, it will be essential to decipher customized solutions that resonate well with the patient’s situation.
