Employers prefer to recruit the most skillful, competent and experienced professionals. Besides, the education level and accomplishments are vital elements valued by the prospective employers. Given this, there is need for the potential employees to prepare all these elements in their portfolios to stand better chances of being recruited to their dream organizations (Why you need an electronic professional portfolio, 2016). These elements can either be prepared, stored and presented manually or electronically. The electronic form is known as e-portfolio. The form has gained traction in the recent past as it is associated with high levels of professionalism and expertise (Green et al., 2014). In view of this, the current report highlights the elements to be considered in my professional portfolio. They include the ones that enhance individuals and operations in promoting individual and group patient outcomes, transition operations to prioritize healthcare quality, enhance collaboration and teamwork in the healthcare environment and integration of professionalism during patient care. 

Competency 1 

Leading People and Processes to Improve Patient, Systems, and Population Outcomes 

The major role of the nurses is to enhance the welfare of the patients regardless of the conditions that they are subjected to. Given these, the approaches used to arrive at this outcome should enhance the most appealing results. I possess many that enable me to offer the best services to the patients that I encounter. For example, I possess the skills necessary to offer comprehensive assessments to all the patients under my care. Besides, I possess the relevant knowledge and facts about the vital signs among the patients and this enhances the overall patient outcomes.

Proper patient assessment is the basis of patient centered care. When handling the cases of the patients that I monitor, I embark on thorough assessment as I get facts from their interviews and medical history. Through the thorough assessments, I obtain the required information about their social influences, health risks and other general risks that are associated with their current health conditions. Markedly, the patient files enhance proper and personalized care as they highlight from the simplest to the most comprehensive facts about the patients. After the comprehensive assessment, I make it mandatory to monitor the patient conditions to prepare for a relevant care plan and to identify vital signs. 

Competency 3 

Transforming Processes to Improve Quality, Enhance Safety, and Reduce the Cost of Care 

Cost issues are common in the healthcare sector and in most situations, low costs are associated with poor healthcare services. My main aim is to alter this notion by offering the best services even in the most unlikely situations. When handling the conditions of my patients, I am driven by the motivation to enhance both quality and safety in their environment. Therefore, to reduce costs and enhance their satisfaction, I embrace evidence-based mechanisms and relevant healthcare technologies that have proven effective in the healthcare environment. The evidence-based mechanisms ensure patient safety, while the healthcare technology enhances the quality of the services offered to the patients through the relevant interventions. In the end, the patients get the satisfaction from their respective healthcare facilities. 

Competency 6 

Collaborating Inter-professionally to Improve Patient and Population Outcomes 

Collaboration in the healthcare environment enhances the patient outcomes and those of the population as a whole. The positive results are attributed to the common goals adopted by the healthcare practitioners. Besides the welfare of the patients, these collaborations are avenues of sharing relevant knowledge and skills. During my course of work, I have embraced teamwork and I contribute positively as I value being an active member in such situations. I find the opportunity to learn and interact with other members during ward rounds and this enhances my knowledge in planning and caring for the patients. Currently, our team has come up with different mechanisms to enhance the comfort and welfare of the patients as it is the guiding force in our operations. For instance, to curb the element of alarm fatigue, we have developed smart alarms that minimize the fake alarms in the healthcare environment. 

My place of worked embraces team-based techniques where different parties are given different tasks. The approach enhances responsibility and accountability, which in turn boost the safety of the patients and improvement of overall care. Ultimately, team-based care enhances proper communication, respect and efficient coordination of services. 

Competency 8 

Integrating Professional Standards and Values into Practice 

The nursing ethics and professional code of conduct are the guiding elements in the nursing profession. In my service, I have always integrated these aspects to enhance the welfare of my patients. The adherence of these guidelines enhances careful actions at the right times when handling the patients (Bevitt et al., 2016). I hold the dignity of the patients highly and ensure that their rights are respected at all times. Furthermore, as I attend to the patients, I embrace the attribute of patience and ensure that I answer all questions from the patients to satisfaction. For instance, I offer the required details about treatment alternatives, costs and the risks associated with given treatments. Ultimately, I gain the trust of the patients as they embark on their treatment journey. 

Also, the culture of competency enhances the healing process and communication of the patients as they embrace the example set by the nurses in focus. Markedly, my approach towards them enhances their welfare alongside our bonding. The bond eliminates the challenges that are associated with the patient fears and as such, it impacts positively on the type of interventions incorporated during their healing journey. Equally, the patient families have peace of mind knowing that the patients are in good hands.
