Methods of collaboration and consultation
In our rounds, we experienced three types of collaboration in the departments and they are as follows.
Interdisciplinary Teams
This kind of consultation involves only one professional field that had direct interaction with the client. In the hospital rounds, the nurses were seen to be the major ones carrying out this type of collaboration. The registered nurses consulted among themselves while communicating with the patients to facilitate them through the care provision. This happened particularly during the handing over of shifts. The same was also witnessed with the nurse educators, preceptors, and patient care specialists.

Interdisciplinary Collaboration
This type of collaboration involves a group of professionals from different fields that work together to support and resolve the client’s issues that are complicated. The same also focuses on the patient. In our rounds, we witnessed such in the consultation between the admitting doctor, physical therapist, radiologist, and the nurses. This was often witnessed in the case of taking care of long term or chronic conditions. The different professionals worked interdependently in the same setting with both formal and informal interaction.
Multidisciplinary Collaboration
In some cases, there was an interaction between different disciplines for the team to offer a greater breadth of services to clients. They worked independently and the interaction was formal. There were projects within the hospital that were aimed at increasing the population health and thus required the input of different professions. They focus on a specific project, for example, the treatment for breast cancer. In this case, the team comprised surgeons, medical oncologists, radiation oncologists, pathologists, radiologists, and many others.
Monetary Resources For The Provision of pt. care
The hospital uses the model of value-based reimbursement. Money is paid through the patient outcomes. This is achieved through the implementation of EHR and software that track the level of care instead of the volume of treatments. This information is sent to the government and other private insurers. The organization of the human resource is such that care is delivered throughout the cycle of the patient trajectory to recovery. The billing for the human resource takes place according to the level of profession and degree of work.
Observations Made In Each Department
Central processing department
The members of this department were responsible for the decontamination, sterile storage, and distribution. In the decontamination area, the reusable supplies, instruments, and equipment were cleaned and decontaminated either mechanically or manually using chemicals. After that, they move to the assembly and packaging area where they are prepared for issue or storage.
Radiology Department
The radiologist was responsible for performing medical imaging to diagnose and treat diseases and injuries. The several images produced include x-rays, ultrasound, computed tomography (CT), and magnetic resonance imaging (MRI). They also help in harmonizing the findings of the images and other tests.
Laboratory
Routine laboratory tests are carried out to check whether there is any change in the patient as the professional compares the results to the previous tests. They do these using samples from blood, urine, and body tissues and subject them to some examination.
Respiratory Therapy Department
The routine of this department is that its members perform modalities of respiratory care in the treatment of pulmonary diseases. They employ the tools to diagnose the severity of the respiratory disorder in patients and monitor their changes. This includes mechanical ventilation and corrective surgeries.
Interdisciplinary team
Members from different professions work interdependently to provide care to a complex case. In the hospital’s complex pediatric care, there was a combination of several members. CHI’s team of physicians oversees the clients’ care plans and their complex medical needs. Rehabilitation nurses incorporate evidence-based practice bed management while case managers oversee the client’s care plan from admission to discharge.
Physical therapy
The daily routine of the physical therapy department involves restoring function, prevent permanent physical disabilities, and relieve pain. They do this by carrying out exercises, applying ice or heat, muscle stimulation, or ultrasound to help clients who are recovering from injuries or chronic diseases.
Occupational Therapy
This department sees patients to plan their unique mode of treatment to restore their functionality to perform daily operations. The routines varied from exercising to picking things and putting them in a place, working to dress oneself, to improving body posture.
Speech therapy
Speech therapists help clients with their safe swallows strategies during breakfast by applying myofascial release therapy and electrical stimulation. They also test and evaluate clients to determine the treatment course to restore their ability to communicate.
Safety measures
The stairwells and hallways are well lit for clear vision. The medicine cabinets are locked safe when not in use to avoid accidental picking of medicines by patients. The hallways are fitted with support handles to prevent falls in the elderly. The floors are nonslippery and are always kept dry. The needles used have safety caps to prevent punctures.
Infection Control Measures
The professionals observe hand hygiene by decontaminating hands using anti-bacterial soaps before tending to a patient. They wear personal protective equipment like nose masks, gowns, gloves, and respirators to protect their skin, clothing, and respiratory tracts from pathogens particularly during contact with the patient where appropriate. There is the safe handling of needles and other sharp objects. Used needles are discarded after use without manipulation in leak-proof, puncture-proof container. Contact surfaces like doorknobs, sinks, and toilets are routinely cleaned with disinfectants.
Therapeutic Communication Skills
The nurses used attentive and active listening. The professionals hear, process, and comprehend on the words of the clients. The other skill observed is using silence to make the client have ample time to discuss openly their feelings, beliefs, and opinions. The professionals also used open-ended questions to make the patients give deeper and fuller information about their situations. Another skill observed was the use of restatement to make the client understand what they just said. Close to this was the use of paraphrasing to clarify their message. One area that needs improvement is stereotyping. Some professionals still attach several prejudices to some members of a community or culture.
New Ideas Learned
One of the ideas learned was the importance of communication skills when providing care. It is imperative to get information and see things from the perspective of the patient and it would lead to favored outcomes.
Concepts That Were Reinforced
The principles of infection control still form the paramount standard of operation in the medical field. The occurrences of accidents and infections that can be avoided still form the basis of the heightened policy in this area.
Summary of What You Learned During Rotation
The rounds were successful and shed light on the complexity of the hospital setting. I learned that there is a need of collaboration within different departments and professions to help with the recovery of a patient.
