The 67-year old required several interventions to prevent her current situation. First, there should have been monitoring of vitals following the operation combined with proper management of hypotension to ensure good response to treatment and proper perfusion. The assessment of the epidural catheter’s functionality and the correct dosage for the analgesia was not done because no records on the same are indicated. Various aspects of the cardiovascular ranging from auscultation of sounds of the heart, peripheral pulses, and cyanosis should also have been given priority. 

Inappropriate or Completely missed Nursing Processes 

The process of nursing involves chronological steps which include assessment, diagnosis, planning, implementation, and evaluation. Based on the case study, the patient was assessed after they were transferred from PACU. This was an inappropriately done process. Also, their interventions panning of implementation and evaluation was not done until when the patient appeared complicated.

Steps of Delegation that were Inappropriate 

Instructions delegated or rather communicated by the registered nurses was unclear. They should have communicated effectively to deliver the message appropriately. The nurses also delayed on delivering the appropriate care to the patient. On the contrary, the nurses should have taken immediate action and recommended the type of care that would be suitable to the patient. 

How the Outcome would have been Prevented 

To prevent the current outcome of the patient several measures would have been taken by the nursing team. Firstly, during the first hour, there should have been quarterly close monitoring of the vital signs. This should have been followed by continuous monitoring of the above vitals for the next four hours on an hourly basis. Close monitoring would ensure early detection of changes in the hemodynamic status if any. Consequently, proper intervention would have ensued. Lastly, had the proper assessment of the cardiovascular been done, the outcome of the patient would have been prevented.
