1. Explain the composition of the inter-professional team

Patients who seek health care services require different modes of specialized care. Old people for example require professional care and in most cases require multidisciplinary teams. Most multidisciplinary teams are not fully inter-professional and therefore results may not be effective. Formation of inter-professional teams however is effective because the team works for the goals of the organization and not personal goals. An inter-professional team comprises of professionals who have received inter-professional education on new ways of professional practices. Inter-professional teams rarely disagree on goals because in their formation, they always agree on the team leader, their working process, their priorities and also how they will reach their goals. In an inter-professional team setting, there have been identified four stages in building: forming of the identity of the team, storming to find ways of working together towards a common goal, norming to identify their goals and ensure good communication in and outside the team and finally performing the task at hand. 2. In your discussion differentiate between cooperation and collaboration

Collaboration is when medical practitioners assume working roles and work together in cooperation and account for the results as a group as well as setting common goals for the purpose of patient care. Collaboration ensures a good working relationship among doctors, nurses and other medical officers. Cooperation on the other hand is mainly the relationship between a doctor and a patient towards successful healthcare. Doctors are trained professional in health matters and patients also have the knowledge of themselves so it is important to have cooperation so that the patient will feel to be part of the process.

3. How does collaboration promote patient safety?

It is evident through a conducted review that collaboration has a positive effect to patient outcomes and also eases their accessibility to health care. Collaboration is the passing of ideas among an inter-professional team in the aim of reaching a common goal. Inter-professional collaboration is important to a patient because it values expert ideas towards patient care. In collaboration, there are working principles that were identified: partnership, sharing, interdependency and power.

4. What is the role of the nurse leader in ensuring inter-professional teamwork and collaboration and conflict resolution?

The American Organization of Nurse Executives in collaboration with the American Academy of Ambulatory Care nursing revealed how nurse leaders caring for inpatient and outpatient departments should conduct their leadership roles. They gave a basis for each nurse leader to refer to. According to their conclusion, a nurse leader must know how to coordinate heath care in their departments by knowing the population, identify patient’s needs and follow through their development.

A nurse leader also must know the health care providers and conduct assessment of each individual and also to define roles and obligations for all members of the inter professional team.

It is the role of the nurse leader to unite and build relationships with individual in the process of setting up an inter-professional team.

A nurse has the duty to know the technological value and the impact of technology in workflow and also the roles of the technological crew. Technology must also be update strategic and have minimal risk.

A nurse leader should also communicate with the patient and the patient’s family. How a nurse leader conduct services should be in line with the patients’ needs.

Finally, the conclusion was that a nurse leader should engage inter-professional team members. A nurse leader must be involved in the process of selecting a leader and oversee all education and workflow of the team.
