There are a lot of challenges I encountered in my clinical placement at the geriatric daycare facility. Most of the common presenting conditions I encountered in the care of the elderly included limited data on the pressing healthcare needs of the referred cases. In one clinical case of chronic dementia and with a history of depression, the clients medical history was incomplete, which made my follow up become a hectic experience. When I prompted the next of kin about the missing medical history information, the information I got was not satisfactory to make prompt care to the client. Cognitive impairment is one of the secondary conditions that follows chronic dementia (Doherty et al., 2014). In my intervention strategy, I prompted the need for digitizing all the medical history of clients to control the effect that follows the loss of such valuable information which are not digitized. 

Also, in my rotation at the daycare facility, I commonly encountered a problem of initiating or completing conversations as most of the clients under my care had hearing difficulties, which set in with age. The problem of hearing loss made my nursing care plan a nightmare as I could take a long time asking the client important information, which is necessary to initiate the nursing care plan. Many clients under my care all had hearing difficulties (Kogan et al., 2016). To effectively initiate the nursing care, I opted to use sign language to ask for vital information I needed.

To conclude, nurses experience a lot of challenges when caring for the elderly. It is, therefore, important for geriatric nurses to tailor each specific need to individual clients. The elderly need holistic care ranging from psychological support, financial support, and physical support. The nursing care intervention is founded on a holistic care approach from clients to an extended relative.
