An end of shift report refers to a compilation of information that involves the details of patients and their medical status as the nurses end their shift. The End of shift report allows nurses to have a detailed description of a patient recovery journey that helps the nurses to begin their turn to get a better understanding.as they begin new changes. The report is compiled during the last hours of the shift where the recovery process can be established for the next nurse to use. The end-of-shift news is very crucial for the patients considering that it helps in creating a picture of the level of improvement on the recovery process based on the medical status of the patient within the past few hours. When making End of shift reports, nurses are required to be specific on the details of the patients to overcome issues associated with misdiagnosis.

The issue of making End of shift report is a nursing leadership issue considering that the nurse leaders are required to ensure that nurses ending their shifts can make proper reports in a bid to capitalize on enhancing adequate care to the patients. A good nurse leader leads by example and holds the vision of the team. The ability to cooperate as a team is wholly dependent on the strength of the leader to make the right decisions. Nurse leaders are expected to closely monitor the nurses by reviewing the end-of-shift reports to capitalize on accuracy.

Literature Review 

End of shift report is a crucial document that provides essential information regarding the recovery journey of the patients at the End of a shift. According to Tan (2015), the preparation of an end of shift report requires the implementation of different aspects that help in enhancing the quality of information presented. The development of the end shift report raises a significant concern considering that it is essential to focus on the legal, ethical, and cultural considerations relating to the patient and the development of the report. According to Evans, Grunawait, McClish, Wood, & Friese (2012), nurses are required to focus on maintaining their legal obligation during the presentation of the report to ensure that it is accurate and does not violate any lawful right of the patients. Additionally, the report should focus on the ethical and cultural considerations to ensure that the nurses beginning the next shift have a better understanding of the cultural issues facing the patient. The considerations play an essential role in promoting the recovery of the patients effectively.

Four stages of the Project 

Starting the Project 

The Project will take place in the Cleveland clinic Martin health hospital, which is a healthcare institution with different shifts where nurses are required to prepare their End of shift report. The major stakeholders in this Project include the nurses, nurse leaders, and the patients receiving care at the hospital. The Project will focus on investigating the importance of End of shift reports among the nurses and the impact on the improvement of general patient care. To undertake the Project within the given environment, necessary approvals are required to provide the ability to interact with the nurses and the patient in a bid to obtain relevant information concerning the main issue guiding the Project. The capstone project will take approximately three months, which is adequate time to acquire relevant information regarding the importance of End of shift report. The problem encountered in this stage involves the lack of sufficient resources to begin the Project.

Organizing and Preparing 

The second stage involves the development of a plan that will guide the entire project towards successful completion. During this stage, it is essential to address all aspects that are critical in the conclusion of the project to identify factors that may influence the proper acquisition of information regarding the preparation of End of shift reports. Teamwork among nurses is a critical factor in creating a connection among the nurses in different shifts. Thus it is essential to capitalize on strategies that would help in enhancing teamwork when undertaking the Project. A significant problem encountered during this stage involves a lack of coordination between the nurses and the medical practitioners, thereby leading to a situation where the nurses do not obtain adequate information concerning the medical status of the patients. The involvement of the nurse leaders helps in resolving the issue, considering that they have the ability to enhance coordination among the nurses and other staff.

Carrying out the Work 

In this stage of the project, the project team engages in the process of obtaining the required information regarding the reports made at the End of shifts. In this stage, the nurses engage in the actual process of writing the stories at the End of their change in Cleveland clinic Martin health hospital. In this stage, the nurses are put in different shifts depending on the flexibility and availability of the nurse. All the nurses understand the importance of the report holds thus have to be team builders to ensure the stories met the required standards. The nurses in this stage are required to interact with the patients and ensure that they engage in documenting proper reports. The project team participates in the analysis of the progress of the Project in a bid to determine its success towards understanding the importance of End of shift reports.

Closing the Project 

The last stage involves assessing the results of the Project based on the information obtained in the previous stages. Results from the Project show that the end-of-shift reports are essential in the Cleveland clinic Martin health hospital. The project results show that all the nurses should work together as a team to ensure that the stories are documented adequately to promote the overall wellness of the patients receiving healthcare services. The results indicate that the nurse leaders have a significant role in improving effectiveness in the compilation of reports at the End of their shift. In this stage, a Post Project Evaluation is necessary to access each stage based on the findings obtained.

Project Results 

The results of the Project indicate that End of shift report is a crucial aspect that requires proper consideration to ensure that nurses have adequate information concerning the patients and their medical status. The results from the Cleveland clinic Martin health hospital indicate that the implementation of proper measures when documenting the End of shift report helps to ensure that a healthcare institution has established standards to deal with the medical conditions of the patients at different times considering that the stories help in providing the healthcare progress of the patients. Nurses have a significant role in ensuring accuracy in the reports to promote understanding and efficiency in the provision of care to the patients.
