There are several problems that individuals with enduring pathologies encounter. From the case, it can be noted that the patient is suffering from a protracted kidney ailment, as well as other related conditions, such as high blood pressure. The care for acute illnesses should encompass a wide array of professionals and aims to provide successful and all-inclusive therapy (Levey and Coresh, 2012) . The objectives are a unique attribute of caregiving since they provide a parameter for the health practitioners and physicians. The SMART methodology is considered the most skillful way to formulating realistic and achievable objectives. Moreover, the approach makes it possible for the assessment of success in achieving the expressed goals. In this sense, the provision of health care for people with acute kidney disease should be holistic and in line with SMART objectives to ensure the patient receives the most efficient and excellent healthcare.

The patient often encounters physical challenges that incapacitate the level of movement. This challenge entails a blend of the patient’s health condition and age. These dynamics usually lead to difficulty in movement due to the dilapidation of the muscular-skeletal system. The mediation required in such a case is aimed at the reduction of the identified combination of factors to improve movement. The expected outcome is the lessening of weight since corpulence plays a significant role in limiting movement. Also, there should be constant checking of the patient’s health, typically conducted on a weekly basis through the use of BMI to observe weight. The aim here is to ensure that the BMI is within the standard range, and the BMI value of the patient defines the success of the intervention. The objective is attainable through the active physical workout and nutritional monitoring (Levey and Coresh, 2012) . This is commonly accomplished within three months. There are numerous means of achieving the desired results, one of them being the use of yoga to improve the patient’s mobility and improve the mental status.

The patient’s mental status is another area that requires attention through intervention. Having reported that he is experiencing depression, it is important that a psychologist examines him. As aforementioned, there is a need for multi-disciplinary intervention to ensure that a holistic approach is undertaken in resolving the patient’s health problems. The aim of the psychological intervention is to relieve the patient’s depression levels, and this would assist him to stop abusing alcohol. The evaluation of the intervention’s success will be through the formulation of a diary that measures the quantity of alcohol intake on a daily basis for one month, and on a weekly basis over two consecutive months (Levey and Coresh, 2012) . The objective is achievable since the known source of the patient’s alcohol abuse is depression. Therefore, by tackling this problem, it will contribute to the patient reducing the amount of alcohol consumption. The goal is to lessen the rate of alcohol consumption to one beer or none over a period of three months. The ultimate assessment will be an evaluation of the diary after three months, and the success will be the reduction of alcohol intake. Apart from psychological intervention, there exist other measures that may be taken to improve the patient’s emotional status (Brooker & Waugh, 2013) . Yoga is significant in improving the mental and physical condition of the patient. Therefore, the health caregiver may also choose to include mediation to improve the patient’s emotional status to ease depression. 

If the patient lives alone, it is recommended that he should make an effort to interact with other people to avoid depression. Depression can be as a result of loneliness, which makes the patient susceptible to negative thoughts. The likely outcome, in this case, is to engage the patient by ensuring he gets into social circles to increase his level of interactions and communication with other people (Brooker & Waugh, 2013) . The patient should be encouraged to go to public places such as parks, where he may achieve a wider social circle. Additionally, there is a need for the patient to keep in touch with other individuals with a similar condition. The health care practitioner should have a database of support groups from which the patient can connect with other people with the same situation. Interactions with other people that are facing the same issues benefit the patient as it creates an atmosphere in which they can share ideas and empathize with each other. Thus, introducing the patient to support groups of people with acute kidney disease will contribute to the improvement of the patient’s physical and emotional status (Levey and Coresh, 2012) . It can be noted that there is no known parameter to assess the impact of increased social interactions of the patient. One of the ways in which the impact could be evaluated is through asking the patient to document his interaction with other people and indicate how he feels about it. 

In the long run, there is an increased need to handle all the problems affecting the patient from a holistic approach. Patients who are of age and suffer from chronic conditions have diverse needs and, as such, the caregivers should be able to formulate attainable goals that will act as a guideline for the methodology of treatment by the medical personnel.
