The patient-centered is the practice of caring for patients as well as their families in valuable and meaningful ways to the individual patient. A patient-centered approach to nursing emphasizes the patient's goals, desires, wants, and needs as the core of nursing care and process (Delaney, 2018). This means putting the patient's needs above the ones which the healthcare professionals have identified as priorities. It entails involving, informing, and listening to patients about their care.
There are eight principles of patient-centered care, which I would use in my provision of nursing health promotion services in a rural community. In implementing patient-centered care, I will ensure that I respect the values, needs, and preferences of the patients. Also, I will work towards coordinating and integrating care to alleviate the illnesses and suffering in the patients (Delaney, 2018). Also, I will make sure to know the patients as individuals, provide meaningful care, foster trusting caregiving relationships, emphasizing freedom of choice, promoting emotional and physical and involve the patients' family and friends, as fitting.

Part 2
An example of a patient-centered care approach which I could offer to a specific rural community would be creating a supportive environment. I would provide care in a healing and comfortable environment which would provide support and peace. The core of patient-centered systems is the real environment. Thus, I would create a healing environment for patients by activities such as relaxing music and therapy programs. I would also ensure the patient rooms are pleasant and available facilities (Pender, 2011). The physical comfort level would comfort patients experience and thus result in higher progress and satisfaction levels. Such programs would include pain management coaching, help with day-to-day living needs and pleasant hospice environment.
Part 3
The master's prepared nurse may assess the patient-centered care systems in the rural residents through the use of a health promotion model to assess the level of patients' well-being. The nurse would assess the behavioral outcomes on the patients, to know whether these behavioral outcomes could have led to enhanced health, improved functional ability, and improved quality of life (Pender, 2011). By identifying the behavioral outcomes, the nurse would now the health promotional behaviors are contributing to the desired behavioral outcomes.
