Population Health Management: What It Is and How to Do It

Healthy people framework 2020 is a federal government prevention framework which was developed by an aim to create a healthy nation. It aims to identify preventable health threats and come up with goals and strategies to address them. As a healthcare manager, adopting healthy people, 2020 preventive framework would help in addressing issues that threaten the health status of a populace. The essential vision of the healthy people 2020 is to provide high quality attain high-quality, longer lives free of preventable disease, disability, injury, and premature death; achieve health equity, eliminate disparities, and improve the health of all groups. As a healthcare manager, developing a healthcare institution that addresses the Healthy people 2020 objectives would be the pillar of building a reputable healthcare institution that is people-oriented. 

Healthy people, 2020 has critical objectives such as addressing healthcare services, addressing issues facing vulnerable communities, occupation health, and global health, among other issues that challenge the health status of the community in the present world. Using this framework thus would help a health system manager prioritize areas of health that matters most to population and address them in their healthcare framework. Besides, the Healthy people 2020framework provides a set of data and information on areas that matters most to the populace health. Using such data to improve on hospital management and operations would ensure that Healthy people 2020objectives are met, and this would help in dressing health issues that are important for the served population (“ Health Access for the LGBTQ+ Community,” 2019). 

Christ Hospital New Jersey is an example of a hospital that uses the Healthy People 2020 framework to address issues that matter most to the health of the community. In their website, the hospital prioritizes issues such as vulnerable group lesbian, gay, bisexual, transgender, and queer (LGBTQ). In the Healthy People 2020 framework, those in authority should prioritize such issues.
