Describe Two Trends in Post-Acute Care (PAC) 

The first trend in PAC is the adoption of patient-centered care. PAC facilities are striving to ensure that the needs and expectations of their patients become the driving factors behind their operations. Hospitals are putting in place several strategies to promote patient-centered care. For instance, many organizations are forming strategic partnerships and multi-stakeholder alliances that strive to offer a wide range of services (McHugh et al., 2016). Practitioners are also adopting care that is aligned with the preferences and choices of patients. The involvement of families and friends in decision-making is another initiative that promotes patient-centered care.

The second trend in PAC is the initiative of government agencies and care facilities to reduce the cost of palliative care. More care facilities are adopting new models of payment that enable risk-sharing and focus on value-based care. Patients also prefer outpatient services to lower the cost of inpatient care. By expanding Medicare and Medicaid, the government further tries to shift the financial burden from families.

Two Typical Characteristics of Revenue Cycle Management (RCM) Teams 

Firstly, RCM teams involve multidisciplinary professionals. These teams track the financial process in healthcare facilities, from patient scheduling, registration, medical coding, billing collections, and claims management (Mindel & Mathiassen, 2015). Thus, the group needs to have professionals from diverse sectors who collaborate to ensure effective management of the revenue cycle.

Secondly, RCM teams unite the business aspect and the healthcare sector of hospitals. The group collects administration data and personal information, including the name of the client and the insurance provider. It then compares the data with the treatment that patients receive. For successful, RCM teams must maintain continuous communication with insurance providers and practitioners.
