Our organization is currently performing home and community programs that involve nurses attending to patients within the community and home settings. Primarily, today's generation emphasizes the holistic approach to health, as they seek ways to improve their wellbeing. This program is based on the patient-centered care approach while highlighting the relationship between patients and healthcare professionals ( Delaney, 2018) . Patient-centered care focuses on respecting a patient's values, communication, and the need to provide care and support encompassing the patient and the family. Patient-centered care is flexible and designed to meet a patient’s needs enhancing the quality of healthcare. Therefore, the program is an independent measure of the quality of services offered in a healthcare practice (Delaney, 2018). The home and community program influences the organization's bottom line by reducing re-hospitalization, congestions in the emergency unit, and improving patient's self-care abilities. 

Patients feel more aware of their well-being life by having follow ups from their Medicare service providers. For example, the elderly, through the home and community program, receive services such as home nursing, personal care, and transportation. Patients under the home and community program, including the elderly, have an advantage since, in the long-term, the program improves balance and activities of daily activities (Lewis et al., 2017). Around the community, patients with cognitive impairment are at risk, especially older adults. Therefore, home and community care programs reduce the risk of more people having this health condition by providing knowledge on identifying the symptoms of the disease. Implementing long-term community-based programs requires trained supervisors and executed for a long time. Moreover, this program requires an extended follow up after hospitalization and having nurses take care of the elderly. However, due to increased technology and access to the internet, health professionals in our organization reach large groups of patients through mobile applications and telehealth, where people get information and knowledge.
