From the Sentinel City demographic summary, the target group for my practice is the senior citizens aged 65 years and over, who account for about 10.5% of the entire city population. Looking at this population, they mainly undergo various physical and physiological changes, some of which render them prone to infections and illnesses. Additionally, Sentinel city has some Veteran population, and based on the data, some of these veterans may include the senior population. However, for this discussion, the veterans shall be treated as senior citizens.
Senior citizens are prone to a myriad of illnesses. Fuschs et al. (2013) point out to hypertension being the most prevalent health condition that affects the elderly, accounting for up to 55.1%. This is then followed by lipid profile abnormalities, which accounts for about 40.4%. In the latter indicator, cardiovascular diseases are among the leading cause of death in the United States. It is, therefore, essential to keep a keen eye on these indicators.

The International Classification of Functioning, Disability, and Health (ICF) model shall be used as the primary health needs assessment model for screening the senior citizens and conducting surveillance on the health indicators of this population. Abdi et al. (2019) acknowledge that many older people have unmet healthcare needs due to challenges in the care system. The authors acknowledge the WHO developed the ICF model as the most effective in meeting the needs of the elderly. Unlike other models, the ICF framework is explicitly developed for the elderly and accounts for all the needs of this population group. From the website, senior citizens are often characterized by compromised nutritional status, thus need supplementation of critical micronutrients to be able to meet their daily needs.
