Records patients note electronically and record family history data are the two menu objectives that provide providers flexibility in phase two. A meaningful use program is an initiative that focuses on transforming healthcare facilities to use electronic health records in exchanging information between the health care providers and the patients (Registered Nursing, 2020). The origin of meaningful use can be traced back to 2009. There was an enactment of American Reinvestment and Recovery (ARRA) and the Health Information Technology for Economic and Clinical Health (HITECH). Meaningful is needed because it improves the efficiency and effectiveness of service delivery in the health care sector by reducing disparities. EMR, EHR, and ePHR are all related to the clinical information system. They all keep the patient's information and make the retrieval fast, reliable, and comfortable (Center for Disease Control and Prevention, n.p). A nebulizer is the most effective technological device that can be used in the management of asthma. The device enhances the breathing of the patient. However, the patient does not use the machine more frequently but only when in need.
Telehealth is a solution to the various health access problems in the community and the future. Telehealth uses mixed electronic media in the delivery of services and information at the comfort of the patient's home. One advantage of telehealth is an increase in patient monitoring and engagement. Also, there is an improvement in the clinical workflow, increasing the efficiency of operations. One of the disadvantages of telehealth is the limitation of physical examination. Some medical health procedures require physical review, but telehealth does not allow that to occur. The designing and implementation of telehealth are costly and expensive. Telehealth programs will be useful in the future, especially when there is a pandemic and there is a need to limit contact between people. The patient has asthma. Currently, telehealth is the best alternative as there is the COVID-19 pandemic that is affecting many operations across the globe. However, the patient prefers to be seen by the primary health care physician to manage the asthmatic disorder. It is a good idea, but telehealth will save the patients time while visiting the hospital and provide them with ready access to health care services. She will not have to visit the hospital and wait in long queues to see the physician.

Few studies have established the connection that exists between social media and the management of chronic diseases. For social media to be an effective medium for managing chronic diseases, there is a need for an intervention that will focus on the patients' individual needs. There is a positive relationship between social media and chronic disease management (Merolli, Gary, and Martin-Sanchez, 2013). The positive connection that arises from the flow of information is the most efficient and effective way. The internet has created a platform where one can easily access medical healthcare services without physically going to the hospital. The act has increased the number of people accessing the services, and the outcomes are massive. The patient does not use any social media website to obtain information about her problem. Instead, she prefers to contact the primary healthcare physician for more information and medication. It is a good strategy for the patient to use as social media can be deceiving and misleading. Not all the information on the various social media platforms is correct and accurate, especially regarding health matters. Some websites provide false health care information, and instead of improving an individual's health care outcomes, the condition may worsen. Thus, it is always appropriate to contact the doctor for further assistance.
Also, there is the issue of functionality of the websites that need to be considered. The various social media platforms' functionality is one of the factors that contribute to access to healthcare services and outcomes. Not all individuals suffering from chronic diseases are young and can easily navigate through social media platforms. Some are old, and the system needs to be created so that its functionality allows everyone to maneuver through easily. The woman is old, and his age might be why she does not use any websites to seek more information about the condition. She might be facing challenges to navigate through some of the websites. There are some trusted health care websites that, if utilized appropriately, the websites can improve an individual's health care outcomes.
Determining the characteristics of an age-friendly community is essential in reducing the barrier of documentation on the elderly of interventions. The elderly in society needs as much support that they can get from the community through various resources. The support ranges from supporting their health to increase their social interactions. There are several community resources for elderly adults that enhance the comfort they get for the community. One of the resources is a park in the community. The park allows older adults to interact with others, interact, and even carry out exercises. Another resource is senior centers. From the centers, the older adults receive services such as volunteers to assist them, transportation to the doctor's appointment, and delivery of home meals (Smith, Lehning, and Dunkle, 2017). Area Agency on Aging (AAA) is the other resource that assists the elderly in the community with adult day care and various health promotion programs. The patient does not attend any home care, physical therapy, or dialysis to manage the condition by herself. The fact that she is old raises a concern as there is a need for frequent check-ups by the physicians. She might address the situation, but a check-up by the physicians will enhance the health care output.
The majority of the community's older adults are receiving help from their children who work in different towns. Hence, in most instances, older adults are alone, and they need all the help they need. Since they cannot care for themselves, their children make their contributions to the various community resources that, in turn, assist the elderly with their daily operations (Smith, Lehning, and Dunkle, 2017). For instance, older adults face challenges in driving, and they have to attend doctor's appointments. Since their children cannot leave work to come and drive them to the meetings, they make payments of the community resources, transporting the older adult to the hospital. The patient has to options while visiting the physician. She can either use public transport or have arranged transport to see the doctor. Support groups are the other vital resources that can be used to enhance patient health care output. The patient uses a health line where she can communicate with the nurse when in need of help. A culture such as religion can influence the health care output of a patient. The patient goes to church, but the culture does to have any implication on his health condition.
Part 2
My experience with the practicum was engaging. I met with a grandmother for an assessment and realized that she is suffering from a lifelong asthmatic condition. As a nurse, there are strategies that I intend to use to improve the health care condition of the patient. I will ensure that a housebound elder home care patient has access to the needed medication and food by bringing the services. Instead of the elder to head out to the hospital and seek the necessary medical services, I will visit them at their home and offer them the health care service. The strategy will be vital in improving health care service delivery. I will recommend the food that the house bond elder should consume and have one of the caregivers deliver the food to her doorstep.
Evidence-based practice shows that the patient's condition is manageable if she stays away from allergies and avoids cold weather that exacerbates the situation. Asthma is a condition that is mainly triggered by other factors such as cold weather. If it is possible to prevent the triggers, then it is possible to manage the situation. There is a need for the patient to be taken through a home-based care training program to manage the condition effectively. The training will provide information about how best the patient can take care of themselves by avoiding environmental triggers. Also, they will receive training on the best action to take in case the condition worsens. The process and exercise of exploring the effect have been impressive since I learn new things. A key strategy is to start by identifying community resources that can assist the patient in dealing with the condition. The patient's safety is categorically established by the resources that can be of assistance in case there is an emergency. The system's cost is determined by the medication, the service fee charges, and even the transportation cost to and from the hospital. Addressing the problem change is not from my experience but because of the interest that I had.
My fundamental interest was in the patient-nurse relationship and how it affects health care outcomes. I was interested in knowing if the physician visits the patient to check on the always has to go to the hospital to see the physician. If it is possible to increase the patient-nurse relationship and reduce rehospitalization, then applying the theory will make the change permanent (Shoghi et al., 2019). For instance, there will be a continuation in creating change through various methods. Such methods include continuous and regular training of the research nurse to improve their skills and roles. Exercise of the physicians will make it possible for them to understand their contribution to improving the patient health care outcomes through a good relationship. Frequent visits to the patient's home will be appropriate ad useful in tracking the medical intervention's progress for the old. The good patient-nurse is very important as most of them are alone at home as their children work in distant cities. They need someone to check on them, and some might even have dementia making them forget to take their medication.
