They have substantially shaped my skills, attitude and knowledge. The new insights and exposure will immensely influence my career growth trajectory. The course contents on case management and home care contained vital lessons for a standard practice and have been quite educative. The two areas therefore have increased my knowledge in connection to case management and home care and have prepared me adequately for the different roles that I will partake after graduation. Interaction with various concepts has been imperative in enabling me achieve the course goals.
One of the course goals purposed to equip learners with skills that enable them to participate in activities that support leadership, career advancement and personal development in advanced public health nursing. The course materials have been essential in enabling me realize the aforementioned goals by emphasizing on need for lifelong learning, importance of engaging in professional development to earn and maintain professional credentials that will be pivotal for my career advancement in advanced public health nursing. The syllabus has underscored the need to participate in public health nursing learning opportunities and take part in education opportunities that enhance interdisciplinary awareness. I have acquired knowledge that pertains to the leadership role played by case managers. The emphasis has instilled in me critical leadership attributes that are vital for realization of the course goals.

The second goal aims at enabling students develop skills that are important in provision of culturally responsive care to targeted populations, communities and individuals throughout nursing process. The syllabus content has reiterated that since case management and home care encompasses interaction of clients and their families in a bid to offer effective coordination of care, healthcare providers should demonstrate an advanced understanding of a patient’s culture by respecting and accepting cultural differences. Courtesy of the course materials, I am now well capable of delivering care that is congruent with a client’s culture. The syllabus has also raised my awareness on disparities in healthcare, an exposure that has been pivotal in achieving the course goals. The materials on case management strategies have enlightened me on the essence of effective application of system based practice in bolstering patient outcomes and safety. I have developed knowledge pertaining application of systems thinking particularly in home care, in improving screening practices and in increasing access to public health.
An additional course goal entail enabling learners to utilize and synthesize reliable and valid data to support health strategies. The course has enabled me achieve the goal through increasing my knowledge on need for data collection to deliver tailored care to populations. The syllabus also purposes to train students on ways to seek feedback from peers and colleagues and the essence of engaging in self-reflection to determine opportunities and strengths for improvement of public health nursing. Involving colleagues is essential in determination of optimal and effective coordinated care ( Stanhope et al., 2019) . The course has enlightened me that engaging peers and working collaboratively with colleagues improves wellbeing of clients and families. I have learned that self-reflection enable care providers to recognize their strengths and weaknesses, improve motivation and quality of care provided. The new insights have been critical in achievement of the goal.
The course intends to instill new knowledge on incorporation of ethical standards of practice as well as professional and legal principles during interaction with communities and in provision of care. I have learned that ethical and legal issues may arise due to limited resources and can be triggered by an increasing need for healthcare services. According to CMSA (n.d), limited resources precipitate concerns related to fair resource allocation with regard to social economic status, gender, age and client’s autonomy. I have learned the importance of adhering to system rules as a case manager by providing services that match the client’s health needs given the resource constraints. I have been exposed to ethical and legal concepts such as breach of contract, negligence, confidentiality, abandonment and denial of services and their consequences if violation occurs. The new knowledge acquired on aforementioned concepts that pose ethical and legal issues in home care and case management have been important in enabling me achieve the course goals.
The course sought to equip learners with skills relating to provision of leadership during care delivery, importance of working in teams and utilization of informatics effectively. Effective leadership is crucial in driving healthcare changes, development of strategies and behaviours for healthcare service improvement ( Harkness & DeMarco, 2016) . I have learned that teamwork is essential in problem solving and in accomplishing shared goals across various settings. The new knowledge has changed my attitude on teamwork since going forward, I will engage more in collaborative relationships during care management to promote positive patient outcomes. I have learned that effective use of informatics allows healthcare providers give patients information, share information with family and coordinate care to improve client’s satisfaction. The emphasis on the concepts has played a key role in achieving the course goals.
The course looked forward to enable students learn the application of critical thinking and interpersonal communication skills during complex interactions and work processes. Case managers’ apply critical thinking continuously when interacting with clients and their families ( Frankel et al., 2018) . In have learned that case managers are obligated to apply critical thinking in organizing and analyzing information so that healthcare problems faced by communities are understood and to make solutions purposeful and outcome oriented. Case management involves communicating effectively with clients and families (Clark, 2015). I now understand the interpersonal communication skills required during home care such as active listening and openness. Application of the skills will enable me establish trust with clients and families, promote rapport as well as elicit cooperative responses to treatment interventions.
The course also intended to bestow students with skills that enable them participate in evidence based processes and provide care in a safe environment. I have learned the essence of cultivating the spirit of inquiry, asking clinical questions, searching for evidence and critically appraising the evidence in a bid to provide evidence based care. I have acquired skills related to building a rapid response plan, developing safety compliance plan and practicing patient-centered care that enhances safe practice during home care visits and in case management.
Conclusion
The course contents have provided meaningful exposure to new knowledge, principles and concepts which have been key enabling me achieve course goals. The insights have supported development of new skills and shaped my attitude on effective care delivery. I am now well positioned to engage in a collaborative process that addresses clients healthcare needs through provision of cost effective and quality services.
