Help her navigate the entire health care system and ensure she negotiates, locates, and engages in services.

Describe three teaching resources 

The list of clinical training or coaching to be utilized in this case will include links to several and variety of websites from all over the world that talks about the topics such as effective clinical instructor, effective clinical coaching and training skills for community-based instructors ( McGrady & Moss, 2018) . Other resources to be utilized for this training include various monographs talking about the topic of clinical training or coaching, and the final resource to be used is clinical education references and journal articles from EBSCO, Google Scholar, etc. 

Identify the teaching strategies that can be used 

The four strategies to be used in training or coaching will include: 

Assessing the patient’s health literacy, this will enable the trainer to assess her current knowledge and skill level which identify whether she will be able to acquire, process and understand the primary services and information from the trainer that would help her make appropriate decisions concerning her health. 

Utilization of her teach-back, this will enable the trainer to explain to her the fundamental self-management approaches, concepts, and procedures to her succeed at the end of the training. 

Learning on health technology, various technologies such as computers, mobile apps, etc. will make it easy or the patient to access coaching materials and read and understand even in the absence of the teacher. 

List the specific instructions that may be needed regarding her medication and what adverse reactions to be aware of/and what to do 

The specific instructions needed concerning her medication during coaching include: patient’s demographics, treatment plan, test results, treatment progress, vital signs, current medications, and diagnosis. Moreover, some of the adverse reactions the trainer must know might come from the client include social withdrawal, depression, unable to concentrate in class and becoming forgetful ( Salzman, Collins & Hajjar, 2012) . In order to handle all these reactions, the trainer will need to subject the patient to physical exercise to help keep her active. 

Identify two factors that may negatively influence adherence to the medication and how they can be overcome 

The two factors that might prevent the patient from adhering to medication include lack of social support and cognitive limitations and physical impairments. However, patient adherence can be improved by addressing the patient’s medical stigma and barriers, and mitigating things that may prevent her from taking her medications on time. 

Describe how to include the family 

The patient family can be included in this coaching/training plan by involving them in decision-making and updating them on a daily basis about the patient’s coaching progress. This will make her family members feel part and parcel of the patient’s treatment or coaching process and ensure they provide the patient with social, economic, and physical support to make her active and motivated. 

Provide information on how and when she should seek support and help 

The patient should always seek support when she feels her situation is getting worse and experiences certain strange signs and symptoms due to her new medications ( Scheinbaum, 2017) . Also, the patient should seek support when she is experiencing some family issues such as quarrels and fights that might prevent her from adhering to coaching or medications.
