The community-wide health assessment (CHA) is designed to examine the health status of a county. It involves a state, territorial, local or tribal healthcare system assessment, which points out major health needs and problems through the use of comprehensive and systematic data collection and analysis methods (Issel & Wells 2017). It is intended to help in the shaping of the joined efforts and vision to improve the residents’ health. The community health improvement plan (CHIP), on the other hand, demonstrates how the health department in collaboration with various community partners can work together to boost the population’s general health by deploying various projects and programs (Issel & Wells 2017). It is a long-term effort whose bottom line is to address public health issues as a result of the health assessment and health improvement processes. Benefits of a complete assessment and improvement plan include, but not limited to an improved collaboration of healthcare systems, increased public knowledge on public health, and identification of strengths and weaknesses to tackle quality improvement efforts. Besides, they help in benchmarking of health practice improvements and baselining on performance to prepare accreditation (Issel & Wells 2017).
