Volland, et., al., (2012) suggest that primary care physicians must undergo a variety of training sessions as a requirement of becoming comprehensivists. The primary care physicians (PCPs) upon their elevation get the ability to handle chronic patients with critical health conditions such as heart diseases and chronic pain among others. Firstly, the primary care physicians must be taught the basics of diagnosing common diseases to act as a preventive measure and also to help avoid further complications to chronic diseases. Apart from that, the PCPs should be trained about pharmacology and specifically the way to manage complex situations of pharmacology. They should also be trained about leadership in healthcare to equip them with the knowledge to lead teams of healthcare during joint care by various physicians (Schraeder and Shelton, 2011). Moreover, they should be trained about ethics in the medical field as well as the knowledge on end of life. 

Long term care is characterized by sophisticated and diverse programs which have also changed over a long time. Long term care services face several challenges. The aging nature of the US population is one of the challenges that the care delivery service has faced. This is likely to increase demand for long term care services whereas there are limited resources. Long term care services face the problem of the limited workforce which is limiting their need to offer healthcare services. Long term care services vary in quality delivery in various facilities. This implies that the patients may find themselves receiving poor quality services in the name of long term care services. The cost of financing long term care services is slowly becoming too much for the states and the federal governments. This is due to the increasing population of the aged band subsequent increase in demand for the services.

In conclusion, primary physicians need to undergo training programs such as basic diagnosis, pharmacology, leadership and ethics to become comprehensivists. These training programs equip the physicians in preventive care and also chronic health conditions. Some of the challenges that may face long term care in future include limited resources to handle the increase in demand for care and increased cost of care.
