One key environmental condition in the host community for my nursing school is the high expectations that the members of the community have put on nursing students. Another condition is the increasing amount of interests on students from the host community seeking to enroll to the nursing school. Collecting information to support this observation would be easy during registration of new students. Similarly, interviewing students who interact with members of the community on a daily basis would be an ideal chance to gather more evidence.

Nurses experience quite a number of challenges trying to implement evidence-based practice. Organizational culture is indeed a major reason in this. Most nurses find it difficult to break from the organizational culture to practice evidence-based practice. In many healthcare facilities I have come across, cultural change has been successful in doing away with the barrier. Many organizations are changing their odd organizational culture to incorporate evidence-based practice among in their cultures. This has succeeded in many places.

Ethical Dilemma
Nursing ethics revolve around offering correct, rational and good care to patients at all times. Specifically, nurses are ethically under responsibility to do work with beneficence, respect the self-decisions of a patient and the responsibility to apply justice (Lachman, Swanson & Winland-Brown, 2015). In my practice, respecting the decisions of patients is one ethical standard I struggle to. This is because at times, I have to make a professional decision with no regard to patient’s autonomy.
Care for Elderly
Community health nurses offer a lot of assistance to families on care of elderly. Top among their input is training and educating families on care for elderly (Alderma, 2010). Some barriers that stand against the provision of home care services to the elderly include but not limited to high cost of care. Care for elderly in urban areas offers care providers with vast knowledge from a variety of care service centers. Rural areas however are disadvantaged in lack of information due to digital divide and few care centers available.
