The target audiences for the value-based treatment are the children and youth with special needs in the community. The potential payer organizations include Kaiser Permanente, United Healthcare and Molina (Beaton, 2017). The providers that might share in future risk contracts include United Healthcare Group, the Health Care Services Corporation (HCSC) and the CVS Health Corporation. The following community based organizations can help in pushing the agenda that the hospital wants to implement for youths with special need.

The Lions Club International helps in catering for adults, youths and children with visual impairments. The organization sponsors the Childhood Blindness Projects that provides various resources to children who have lost their eyesight (Young, 2016). This group has been helpful in screening activities and may provide much needed assistance in the value-based payment that seeks to provide better care for clients with special needs. The Easter Seals organization offers assistance to persons suffering from autism (Young, 2016). This group provides training on how to handle persons living with special needs, medical rehabilitation and brain health programs. The United Cerebral Palsy program helps in providing support services for families and youths living with cerebral palsy (Young, 2016). The arc provides individual and family information and referral services as well as residential and family support for people living with disabilities.

In addition, The Friendship circle makes home visits to families with youths with special needs. This allows them to provide guidance and support to people catering for disabled youths which would be essential in promoting the value-based payment model. The Goodwill Industries International is familiar with such programs, and provides financial coaching and community services for activities involving children with special needs (Young, 2016). Partnerships would be developed through maintaining close relations with these groups and taking part in community centric activities that aim at improving the current lifestyle of patients living with special needs. A better health IT model should be implemented to help in gathering, analyzing and reporting the data across the organization (LaPointe, 2017). HER technology is the most effective way of assessing progress and delivering accurate reports to superiors. The HER system also bridges the gap between the community receiving the care and the hospital; hence, they are able to appreciate the progress in delivery of quality healthcare.
