Today, case management services have become integral in the changing healthcare landscape. Healthcare systems like Mercy are focusing on increases operational efficiency and quality through the adoption of case management services. Last year, when my cousin Tom was admitted at Mercy Hospital, he used case management service in acute care, which proved to be effective. In acute care, case management services are meant to improve the patient’s outcome. As the healthcare industry moves towards a value-based reimbursement model, acute case management is vital. According to Shaikh et al. (2018), the efficiency of discharge when applying a case management system does not affect the patient's satisfaction and quality of care provided. Case management helps the healthcare providers to focus more on the quality of care given as opposed to focusing on the volume of services delivered. 

Case management processes involve nine phases that are iterative, recurring and recursive and are applied until the needs of the clients are fully achieved. In acute care medical services, case management care services ensure that care providers are on the same page, enhancing communication between providers and patients and streamlining processes to create efficiency and reduce the overall cost (Fabbri et al., 2017). The first phase is screening, which entails the identification of diseases through tests and examinations. The second phase is assessing the patient's health situation and must cover all aspects of individuals involved in the diagnostic process (Fabbri et al., 2017). The third phase is stratifying risks. Planning of the intervention is the central aspect in the case management process where practitioners seek synchronizing actions and arrange them to ensure effective medication and patient care. The next phase of continuous monitoring entails follow-up, transitional care, communicating post-transition, and lastly, evaluation of the patient's outcome.

The process of case management is well organized to ensure it matches the on-going needs of the clients. Based on the course reading and my Cousin Tom's experience while receiving case management acute care services, I would not change any of the processes. The entire process is well-coordinated, and there is optimal use of resources specifically to ensure professionals issue quality services.
