Self-managed care strategy -This includes techniques initiated by the patient to ensure that he/she is adhering to medication and risk preventive behavior. In the treatment of stroke, a medical practitioner may encourage and propose to the patient to some of the strategies that will help develop a behavior towards the recovery (Caminiti et al., 2017) . One example of this strategy is self-efficacy where the patient gains confidence in his/her actions towards recovery.

Nurse case management-The nurse initiates a program of patient follow up in assessing the progress in the recovery process. The follow up include assessing the medicinal adherence and avoidance of stroke risk factor. The nurse can incorporate interviews and making calls to the patient or family members to get the information on the far the case has reached. The nurse can also assist the case with memory cues like medication dossette every week.

Group sessions-The patient is integrated into a group of patients suffering from stroke and they can share information and experiences in battling the condition. It is highly recommended as it offers motivation from those registering success in fighting the disease.

High blood pressure 

Self-management strategy-This approach involves home blood pressure tele-monitoring (HBPTM) where the patient regularly monitors his/her blood pressure at home to know the progress of recovery. The patient is offered the Automated Home BP monitoring device and he/she can be able to regularly take the readings to know the success of the actions taken.

Nurse case management-In blood pressure management, the nurse employs a variety of techniques which include telephone-based assessment, special websites, motivational interviews and problem-solving techniques (Spruill et al., 2015) . In telephone-based NCM, the nurse makes regular calls to the patient or his/her family members inquiring about the progress made. The nurse or the health facility can create a website where they can post important information for the patient to access or where the case posts vital data like BP readings.

Group study-The patient can be introduced to other individuals with similar cases from which he/she can learn vital information.
