For productive and efficient Community Health Needs Assessment (CHNA), its necessary to implement an eight-step approach to superintend the assessment and implementation process. According to Stouten, Rousseau & De Cremer (2018), t he steps include stakeholder identification and engagement, the definition of the community, data collection and analysis, prioritization of community health issues, documentation and communication, planning improvement strategies, implementation of the plans, and progress evaluation, singly. However, community and patient engagement are central to the entire process. 

The process will start with the assessment by identifying and engaging community stakeholders who are actively involved in family and patient advisory councils. By so doing, trust will be established to encourage the stakeholders to bring in a community perspective to health operations and programs ( Alley et al., 2016) . After that, I will define the community in my location in a bid to determine the scope of assessment and intervention. Subsequently, I will aggregate primary and secondary statistical data, which contains both quantitative and qualitative measures to help in the prioritization of community health needs.

In the fourth step, I will select priority health issues based on the scope and severity of identified health issues, the healthcare system, and community stakeholders as illuminated by quantitative and qualitative measure analyzed in the previous step. I will then go ahead, and document and then share the CHNA drafts with community stakeholders using the guidelines by Stouten, Rousseau & De Cremer (2018) . This process will allow the stakeholders to comment and provide auxiliary feedback about the presentation of CHNA information. 

I will proceed to plan improvement strategies and evidence-based interventions, which will play a pivotal role in resolving health issues within the community as suggested by Alley et al. (2016) . The planning process will involve family and community stakeholders who will later provide valuable feedback about the acceptability and feasibility of the improvement strategies. The seventh step will involve the implementation of evidence-based interventions and improvement strategies formulated in the previous step. At the end of the Community Health Needs Assessment process, I will evaluate the progress of the entire process by reviewing patient outcomes and feedback from families, patients and the community.
