Diabetes is a critical health issue that is likely to impact public health objectives if not taken into active consideration. To achieve a healthy nation by 2030, it is important to focus on several key health and illness recommendations aimed at promoting effective diabetes management and prevention. The first recommendation is on increasing engagement in physical activity within the public sphere. Azzopardi et al. (2012) indicate that engaging in regular physical activity would be of great value in diabetes management and prevention because it boosts one's sensitivity to insulin, which aids in keeping blood sugar levels at normal range. Therefore, health professionals ought to provide recommendations on aerobic and resistance training as some of the critical approaches to help in the effective management of diabetes. The second recommendation revolves around healthy eating, which has become another key issue of concern. Lack of proper eating habits exposes populations to increased risks of chronic diseases, one of which is diabetes.

The recommendation focuses on the idea of ensuring that the population consumes foods that are more nutritious as a way of reducing the risk of diabetes. Page & Reisman (2013) denote that consuming foods that are rich in fiber and taking whole grains may play an essential role in reducing the risk of exposure to diabetes by ensuring that the blood sugar levels remain at normal range. Additionally, healthy eating will also help in weight reduction, one of the contributors to the risk of diabetes. The third recommendation involves regular health checkups as a way of promoting early diabetes detection. One of the greatest challenges the public is facing today is that they have not embraced the idea of early detection of diseases through regular checkups. Most people visit health facilities when they are sick, which affects their ability to manage diabetes effectively. Every person must understand the importance of going for regular medical checkups to avoid negative health outcomes.

Health Care Reform has influenced health care by shifting focus towards specific conditions and illnesses that have serious implications for the populations. For example, through Health Care Reform, diabetes has been noted as a key contributor to poor health, considering that it affects a significant part of the population today. The reform uses statistical information on different health conditions to help push the agenda of dealing with specific health conditions. On the other hand, the reforms have influenced the strategies used as part of the health care industry, focusing on improved health outcomes. One of the key areas of consideration for developing health strategies has been the implications that these strategies will have on the bigger population. Reforms in the health care sector reflect on the need to change tact in dealing with different health issues to maximize health outcomes.

My participation as a member of a multidisciplinary health care team will focus on two main areas. Firstly, I will focus on patient education, one of the lagging areas in advancing positive health outcomes. To ensure that my patients follow the set recommendations, I will develop training and education programs targeted at promoting improved health outcomes. The program's objective is to provide patients with a much better understanding of what is expected of them as they promote efficiency in the management and prevention of a vast array of health issues. Secondly, I will focus on promoting patient-centered care as one of the key approaches to health care service delivery that is proactive. Patient-centered care is a health service strategy that focuses solely on the patient to ensure that his/her health needs are delivered through the plans adopted. I will use this as an opportunity to enhance efficiency in promoting reflective outcomes focused on improved individual and population health.
