An American health care issue is all about the realization of having an assessable and affordable healthcare system for the elderly or the retiring population. The concept of Home care for the elderly patient has been developed into providing critical long-term care. The elderly especially those who are diagnosed with dementia and other terminal diseases can be treated within the setting of the home. The real advantage of home –based care is the cost-effectiveness and personalized care. The institutionalization of home-based care for the elderly has been subjected to explicit decisions that have added advanced to increase long-term care to the aged population. Home-care management programs and institutes are created to offer all in-home based health assessment that assists the elderly and younger persons with disability to live in the home setting of their choice. The programs will help the elderly to transit into the community from the hospital care.

GholdenDayz In-Home Support Services Agency is a professional home-based care agency which supports the needs of the elderly patients by offering a broad range of home-base care services. The agency shall support the need to help the assessment of patients between the age of 65 years and above with the health conditions. The organization was created in support for the affordable, efficient and effective home –base care. The principle is to adapt and be flexible regarding the proficient care in accessibility to care.

Mission. “To encourage collaboration between the Healthcare practitioners and the community in supporting home-cased care in America.”

Vision. The vision is to ensure that our commitment towards an inclusive and assessable social and medical care for the disadvantage in the community.

Value Statement. GholdenDayz In-Home Support Services agency values the partnership with all health practitioners in assessing and providing affordable care for the elderly. Humanity remains in principle to give a solution to health care and social care for all our home-based medical programs. The agency values the strength in education, community participation another social welfare programs. Quality nursing care and other crucial programs will improve not only the life of the disadvantaged but also collaboration with the community as a whole.

We have mechanism and programs that will be useful to improve accountability, transparency and ethical conduct.

GholdenDayz In-Home Support Services Proposals 

Our Target Group concerns. GholdenDayz In-Home Support Services agency target group comprises the elderly population in the country. The number of the elderly population aged 65 years and above is estimated at about 43 million in 2014, and accounted for about 14.5 percent of the population It is expected the population has a double rate increase in the current year due to be demographical changes in the aging population (Mather, Jacobsenand Pollard, 2015). The current growth rate of the aging population of 65 years and above has challenged both the government and the community –base organizations like GholdenDayz In-Home Support Services agency in the provision of affordable care. The baby boomers are significant aging population which comprises of about 20 percent of the population. However, there is the growing concern on the population as many lack inadequate health care coverage while in retirement. The population is not entirely healthier as statistics states that almost every four out of ten aging boomers have conditions in obesity, while many still suffer from dementia, hypertension and high cholesterol. The health concern has made the proliferation of the nursing home, about 2.3 million in number, which provide comprehensive care according to the needs of the population (Mather, Jacobsenand Pollard, 2015). The high number of baby boomers has put a strain in the health care system and increased significantly the demand for nursing care home and the nurses. In reality the baby boomers usually require a specialized care from different specialist. Medical practitioners and the nurses often provide the comprehensive care by to the elderly patients in a nursing home by working with interdisciplinary teams of providers like physicians, social workers, specialized therapist and family members (Carrington College, 2014). There is no present health care system that is designed to diagnose, assess and treat multiple chronic diseases in the aging group. Preventive care should be implemented to reduce the chances for the aging population to reduce the risk of contacting chronic diseases. GholdenDayz has proposes community programs and financial grants that encourage the need for long-term care. The pool of funds will increase the ability to provide affordable and specialized care for the aged in the long-term care. Baby boomers need a community-based delivery system in which successful care for the aged will be significant. The proposal is to include more options on recreation, health care and modification of the village retirement homes based on the treatment needs. The arrangement is to make more community programs that engage with the elderly while giving opportunity for social interaction other than medical-care (Knickman and Snell, 2002).

GholdenDayz In-Home Proposed Services 

In America, almost four out of the seven aged are reported to have cases of obesity and other related diseases and conditions like cancer and heart related diseases. The individuals will require formulated treatment plans at the comfort of the house which is a cost-effective care. The proposal is to provide in-home nursing care-services. The services will include appointing specialized nurses and doctors who frequently visits the patients on a daily or weekly basis depending on the treatment plan (Grady, 2011). The services will both have form of cost savings and have a basis for personalized, well monitored and comfortable care for the patients. The services will include having a specific emergency response teams in the event of medical emergency.

About 10 percent of the total aged population has Dementia related conditions. The condition refers to absence of cognitive reasoning (American Hospital Association, n.d). The condition however is amongst the leading cause of deaths in the aged population. Most families, who have patients with dementia, would prefer a home based care for the needs. The patients are disadvantaged since they cannot have cognitive reasoning hence require a personalized care. The proposal is to have a Home Medical Aid Service. These are services that are not limited to personal care like dressing and grooming, household tasks like cleaning and washing and medical services like checkups and other related nursing care for the patients like in-house therapy session or recommended clinical care for such patients. GholdenDayz In-Home services also propose an in-house assistive technology like aids and equipment that will assist the aged to support daily activities. The technology shall be provided on a cost-sharing basis.

The medical social service will also be provided. These services include companionship and recreational or community work activities to elevate the social needs. In this particular service, it is justified since the agency will improve not only the health of the individuals but also a basis to create treatment plans for the patients based on the cultural and social concerns of the patient. The GholdenDayz In-Home proposes services that have elements of community engagement, such as recreational services, companionship and community works.

GholdenDayz In-Home Financial Proposal 

Currently, GholdenDayz depends solely on the contribution from the client’s payment fees and reimbursement of Medicare. The agency has the goal to increase more services and create awareness to more patients that require the services. The ability to outsource for more resources is needed to achieve the organization’s mission. Services provided are affordable and accessible to all the clients, there is the need to source for more funding and other related resources.

GholdenDayz currently has about fifteen employees comprising of Registered Nurses and Social Workers. We have a specialist home care director who directs all resources towards making sure the patients are served respectfully and efficiently. The only facility available is a single office in New York that was provided by a good-wisher. Supplies and equipment are stored in the same facility, which is used to provide primarily home base care like cleaning and grooming services.

GholdenDayz intends to increase more services and programs across the Country. The expansion will require appointment of more full time nurses to add up the current number of home care registered Nurse and Doctors. GholdenDayz will need part time community workers or care-givers for our companionship and community work services. The costs will the labor budget and other operational costs up to 10 percent more than current budget. GholdenDayz proposes more facilities and offices across the country over the next ten years, which will be in correspondence with the demand for in home care, service especially the companionship and in house medical services. Assistive technology for the aged patients will also be required. The equipment will range from assistive devices like canes, refurbishment of toilets, response technology like emergency sirens and many other assistive devices that will be necessary for the long-term care. The equipment coupled with other supplies will add up about 15 percent of the current budget. As the service grows we anticipated a yearly increase in the overall resource budget close to 10 percent annually, this will include operational, advertisement, labor and financial cost for all the services.

Community Partnership 

YMCA. The Young Men Christian Association is established in almost all neighborhoods. It is a nonprofit organization that is committed to helping people and communities learn and thrive. The organization has programs that engage with the community to improve the health and well-being. As a strategic partner, it will give GholdenDayz In-Home Support Services a platform for its companionship care and recreational activities for our clients to engage in community work while remaining healthy and happy.

Alzheimer’s Association. The association emission is to eliminate Alzheimer's disease through the advancement of research and provide enhanced care and support for the Alzheimer's disease patients to reduce the risk of dementia in the aged population. As a strategic partner, GholdenDayz In-Home Support Services will support the care of these patients at the same time advocate for affordable and cheap care for the patients through collaboration.

GholdenDayz In-Home Support Services leadership 

The board of directors shall be functioning as legal and operational authority for the agency. The individuals who will serve as the board member as will be appointed based on merit and experience in the health care industry. These professionals will have the ability to make concise decision that is in line with the agency's mission, vision, and ethical values in providing a comprehensive care to the aged patients. The membership of the board will include the Chairperson to the Board, the secretary who will be the CEO, the Executive Directors for various services and regional managers. The leadership of the board will have the requisite skill in health care since they will mainly have experience in the health care system, financial skills and competent in analyzing and implementing the legal position in terms of changing medical laws and oversee the management and fiscal affairs of the agency.

Staffing Plan 

Registered Nurse (RN). The staff member shall provide skilled nursing service. The candidate shall possess the Masters in Nursing and a 4-year minimum Home Care experience with excellent communication and analytical skill in nursing. The experienced nurse shall conduct an initial plan for treatment and necessary revisions to the scheme, initiate appropriate preventive and rehabilitative nursing procedure for all the patients. The treatment plan created will include cultural assessment necessary to formulate services that response to the patient's needs.

Mental Health Counselor (MHC) with a Master’s Degree in Counseling, and experience in home based care should have strong analytical skills. The candidate will participate as a member of the patient care staff to formulate the patient plan that includes the mental status and emotional condition.

Community Worker , with a requisite undergraduate degree in community and social work, shall support the companionship services. The requirement is to have a proper assessment of the patient's cultural background to formulate a responsive service to the need. This includes the race, cultural belief, family network and much more. The community worker will be responsible for forming the specialized programs in the companionship service considering the treatment plan.

Benchmarks or Milestones 

The national benchmark for the home health skilled nursing direct cost per visit is at the value of $87 (Centers for Medicare & Medicaid Services, 2016). The cost per visit for the agency is a measure of the financial performance. When the cost per visit is higher than the benchmark, a review the productivity and the payment module for the service will be conducted. The benchmark will put in place strategies that will help reduce the cost of care for the high growth of the baby boomers. These strategies will include having partnerships with the insurance companies to reduce cost module for the home care package.

As a commitment to medical social care, the milestone for the agency will include culturally inclusive, competent and linguistic competent staff members with the important resources like amenities. The milestone will achieve the need for more social care to the aged population. The implementation will include a partnership with nonprofit facilities that provide grants, education, and training to the social worker and create well culturally qualified skilled workers to target the care for a healthy aging group.

Legal and Regulatory Issue of Baby Boomers 

Baby boomers do not have an access to long-term care under the federal law. Long term care is addressed by the need of assistive care at homes. Many baby boomers did not plan or contribute saving for Medicare or Medicaid (Calmus, 2013). However there is the legal issue of the eligibility of the individuals under the two federal health plans. Long-term care is considered non-medical since it supports funding for assistive support for the aged and the impaired for daily living. Medicaid helps the Long-Term Care for the baby boomers even though most individuals do not have the information on the eligibility and the financial consequence of the medical aid. Medicaid is under strain due to the high number of the baby boomers putting a constraint on the issue increasing the cost of the home care (Calmus, 2013). The cost savings for the Medicaid can be initiated by the agency by introducing assistive living facilities that can be developed into independent living residences that provides a more independent care needs. Proposed setting coupled with partnership with the government would incur cost savings to the eligible Medicaid patients through lower cost home settings. The home environment would also form incentives on the possible self-sponsorship for the care through community involvement for financial support.

The Centers for Medicare & Medicaid Services has released the latest rule about that modernizes the Home Health Agency Condition of Participation (Centers for Medicare & Medicaid Services, 2016). The law strengthens the service by the agencies and strengthens the rights of the patients. The regulation focus on assuring the protection and promotion of patient's rights to enhance the care planning tools and delivery of the services, which builds towards the quality of service improved. The regulations will improve the need to reevaluate our services each and every year to ensure the outcome of the services is positive. Client's health records will be protected through confidential forms and assign specific records to third parties for security purpose.

Conclusion 

The information based on this proposal has given solutions for home health care. Our target group is the aging population who demand a specialized and personalized care at the comfort of the home. The influx of the baby boomers seemed an opportunity for the organization to provide a solution for affordable care and require individuals to be aware of their eligibility in Medicaid and Medicare. Cost-sharing is the best means to reduce the strain in health cost for the elderly. The agency brings forth the solution by forming more services at the comfort of the home with close supervision.
