The information derived from the assessment will help to form a diagnosis. The diagnosis will assist determines the primary illness of the patient. The evaluation and diagnosis will form the basis for developing a nursing care plan (Sitzman, 2017). It is under this stage that a care plan goal is set. The goal helps the patient and the nurse determine the best way to make the care plan efficient. Implementation will involve putting the care plan to work, with a view of meeting both the nurse and the patient's goal ( Alejandro, 2017) . Finally, evaluation is done at the end of the care plan. It assists in establishing whether the objectives are met. Evaluation can be done during the implementation or at the end of the procedure. It is done during the implementation stage if the patient’s condition is not improving or getting worse.
