Performance Improvement Initiative
Miami is among the states with the highest obesity cases in the US. The proposed program intends to promote healthy living among the population. Nurses are well placed to offer a wide range of healthcare and preventive services to patients. They can provide health care advice for weight management. Patients suffering from obesity have the desire to lose weight, especially when advised health care providers (Walsh, Grech & Hill, 2019). The proposed initiative is to initiate education on weight management.

The data determinants of success
To assess the levels of effectiveness of the program, certain variable information will be collected to judge the progress of the program. An evaluation process will involve collecting qualitative and quantitative data from the analysis and explanation of the situation. The process will include interviews, surveys within the community. The process will assess the community's views on health education programs before and after its implementation (Zhu, Norman & While, 2013). The view will assess the community lifestyle and levels of participation in physical activities. Evaluating the foods stocked in the market places is relevant to know if they are within the nutritional value standards proposed in the intervention (Walsh, Grech & Hill, 2019). The objective is to reduce the occurrence and parlance rates of obesity with the community. To gauge the attitude of the community towards obesity prevention initiative.
Implementation of the Plan in the Organization
What interdepartmental communication channels will be used for plan implementation?
To improve the levels of collaboration, interdepartmental communication is important. Using the best communication channel to promote the implementation of the program will involve using email for disseminating information. Interdepartmental team meetings would be a good way to update the health care workers and respond to some of the emerging issues (Zhu, Norman & While, 2013). One on one meetings with heads of department will be useful in discussing issues related to the weight management program.
Data interpretation will be used to communicate the findings within the Organization
Data from interviews and feedback from questionnaires are important tools that would communicate the findings within the Organization. One on one conversation will help in collecting information. Involving those who have been part of the intervention will provide feedback.
assessing the program's hypothetical effect(s) on patient care outcomes? How will health information systems support those improvements in patient care
The program outcome is to improve ways of involving the patient in daily conversation to encourage them to take initiatives that will reduce their risk of obesity. Thus the result will be weight loss and improved health outcomes among those who participate in the program.
Hypothetical effect of the quality or performance initiative will be on the culture of safety within the Organization?
Encouraging conversation of weight management is important because it will build a culture whereby the health care providers and patient developing a trusting relationship and adhere to the advice.
Conclusion
When patients receive care advice for weight management, they are likely to adhere to these pieces of advice. The fact that individuals suffering from obesity often show the desire to lose weight but lack information. If initiated, the initiative will encourage lifestyle changes to reduce the risk of obesity.
