This health promotion plan is intended for the community to advocate for healthy living. Living a healthy life is crucial at community and individual level since it prevents or reduces the development of lifestyle-related diseases, genetic diseases and others relating to the environment where a person lives. 

Goals 

The purpose of this plan is to reduce the prevalence of diseases that can be prevented through healthy living. Cases of the throat and laryngeal cancer, COPD and Leukemia have increased within the community because of lack of awareness and exposure to toxins. These diseases can be prevented if the society is aware of their causes and adhere to prevention measures. Another goal of this plan is to ensure all community member is aware of the factors that affect their health by eliminating ignorance and false perceptions (Pender, Murdaugh, & Parsons, 2011).

Objectives 

The primary objective of this plan is to increase awareness of the diseases affecting the community. The other aim is to inform community members of the effects of the chemicals from the factories in their immediate environment. The plan also intends to facilitate increased early diagnosis of these diseases. 

Risk Factors 

Due to the high number of factories in the community, the residents are likely to be affected by related respiratory diseases. Lifestyle is also a risk factor. Smoking, alcoholism, and lack of exercise are among the leading negative lifestyle causes of these diseases. Furthermore, genetic factors may combine to cause diseases like leukemia. Pollutants are modern day’s leading causes of most cancers. 

Intervention 

Exercise is necessary to keep the body fit and also boost the immune response to disease infections. Healthy eating habits should also accompany exercise. Chemicals exposure, which is the likely leading causes of the diseases in the community should be avoided where possible. Workers in the steel plant need to wear protective clothing. Proper nutrition can mitigate the effects of air pollution on the body. Unhealthy lifestyles like drinking and smoking need to be avoided. Actors that prevent healthy living like ignorance need to be eliminated. 

Diagnosis 

Early diagnosis of most diseases is vital in the management of the diseases. It reduces the cost of treatment and also increases the chances of full recovery. Regular medical checkups are necessary for community members. Due to costs and poor access to medical facilities in this community, very few people undergo regular medical checkups. Again, management of the disease after diagnosis needs corporation from the patient. Such commitment should be in the form of adherence to prescribed medication and changes in lifestyle. Offering diagnosis services and checkups at subsidized rates at the facility is necessary. 

Evaluation 

This plan is evaluated based on the set goals and objectives of the program. A good health plan meets set targets within the specified period (Issel & Wells, 2017). The purpose of this plan is to reduce the prevalence of the mentioned diseases. This goal will be measured by the statistics collected on the named diseases after a two year period from the launch of the plan. Awareness, on the other hand, is measured through knowledge of the diseases. A survey to assess the understanding of the diseases together with the perceived barriers to good health behaviors is necessary (Pender, Murdaugh, & Parsons, 2011). The objective of the plan to increase early diagnosis in the community is seen through the number of patient who are diagnosed and their conditions managed. Again, factors that prevent the success of the plan are removed. Thus, this health promotion plan intends to increase community awareness on good health behavior.
