Part 1:
Deficit assessment is identifying the patient’s existing problems and concerns, while strength-based approach is listing on the patient’s assets and work a plan base on these assets. Deficit assessment might be a little bit frustrating and depressing on the part of the patient because it only allows them to see the problems that might limit their performance and productivity. Whereas, for strength-based approach will give the patient an optimistic view of their current condition. It will allow the patients to see that while there are limitations and health restrictions, all of these can be effectively managed if the patient would cooperate and favorably respond to the health care plan that will be specifically created to address the patient’s needs based on what is already readily available for the patient and their families. I believe that what we use in the hospital today is a combination of both approaches. There is the need to inform the patient of what is lacking so they can work on those areas. However, hospital nurses also list the available things that the patient can take advantage of in addressing their issues like a strong support group from the family, a comprehensive health insurance, and their economic stability.

Health Perception and Management
Nursing Diagnosis: Patient believes that while he has existing medical/health issues, he is generally healthy.
Example of open-ended question: What makes you say that you are generally healthy despite all of your existing medical condition?
Activity Exercise
Nursing Diagnosis: Patient thinks that the reason why he suffered a heart attack was because it happened when he was working out.
Example of open-ended question: What did you feel when you had a heart attack and why did you think it was because of what you were doing that caused the reaction?
Sleep Rest
Nursing Diagnosis: Patient refuse to take a break from work and firmly believe that the only way to succeed at work is to get involve in everything and do things that might compromise his health like smoke and drink.
Example of open-ended question: What feeling do you get that you felt drinking and smoking relieves your stress and exhaustion from working.
