The social determinants include language, familial support, economic dependency, health cover accessibility and social support. The barriers that exist are the inability of the patient to comprehend most of the instructions given to (her) due to hearing loss. Also, the patient is allergic to Baclofen which may be difficult for the patient’s family to note and keep track of, especially if the care giver changes/switches.

The patient’s strengths are the non-smoking behavior and the availability of effective health care/medical cover. The family’s strengths are the network of social support that they have created for the patient.

The objectives correlate with the patient’s efforts of seeking better health care for (her) kidney complications, quitting smoking and taking measures to reduce her hypertension, like not taking up more tasks, and relying on family for heavy domestic duties. The patient is making efforts to achieve the Healthy People Objectives.
