In most nations, the incumbent government are usually determined to improve the healthcare service delivery to its patients with the primary reason of increasing their quality of life. Spiritual assessment is generally incorporated in healthcare facilities by medical practitioners with the aim of providing quality healthcare to its patients. Despite the advancement in medical practice, significance of religion and spirituality has been enhanced in healthcare, medical practitioners usually incorporate the religious backgrounds of patients in their treatment plan and not to forget the patients’ spiritual beliefs. The professional bodies have offered guidelines which tend to subject medical practitioners into creating a spiritual evaluation tool in undertaking their medical practice to patients (Yeom, 2017). However, analyzing the multiple resources, I drafted my spiritual evaluation tool for the religious requirements of patients by asking four questions. The interview questions which I have incorporated in undertaking my interview is FICA.
Interview
Do you consider yourself religious or spiritual?
‘’ My spiritual background Is Islam, and it tends to consider myself as a great follower of Allah and the Great Prophet Muhammad and his teachings. The family background that I tend to belong firmly believes in Allah as the majority of them are Muslims. However, there are a few Christians in from our extended family members who broke away from Islam. The strong Islam faith tends to offer me a sense of direction in all activities which I tend to practice in my daily activities (Williams et al., 2016). My friends and family tend to offer me the required spiritual and psychological support concerning my firm spiritual and religious beliefs regarding Islam.’’

What significance do your faith and belief have in your life?
‘’ Islam plays a significant role in building my morals and enhancing social coexistence with numerous people in the society. Islam tends to have strong spiritual readings which tend to play an effective role in nurturing the lives of individuals in the society (Blaber, Jone, and Willis, 2015). Islam also provides believers the guideline in which we Muslims can coexist with other people in the society. The firm Islam believes which I tend to possess usually plays a critical role in how I undertake my living habits (Riley, 2015). Islam tends to boost my psychological morale whenever I feel that I am psychological unrest with my inner self. However, I tend to believe that Allah and his holy prophet Muhammad are the custodians and giver of life and good health in people in the society. Islam plays the role of ensuring that I take care of my body as it a property that is offered by Allah in the society.’’
Are you part of a religious or spiritual community?
‘ In perceptions of self, I tend to believe that the community that I reside in is a community that is from a strong Islam background. Furthermore, I was born and found that my family and the community that I reside following the Islam scriptures. Therefore, I believe that I am part of a strong spiritual society (Puchalski, 2014). According to my opinion, I love my friends, neighbors and my family members as they have been a great influence on my spiritual and healthy life.’’
How do you want me your medical practitioner to address the issue in your healthcare?
‘’ One of the issues that I would like you to address extensively is the way our Muslim women are handled in the healthcare facilities (Kourkouta and Papathanasiou, 2014). In Islam, women are not allowed to be handled by another man who is not her husband. Therefore, the healthcare facility should ensure that medical practitioners who are women manage Islam women.”
Conclusion
All in all, a competent medical practitioner needs to incorporate FICA in assessing the spiritual beliefs of the individual in the society (Gluvas, 2015). FICA spiritual assessment tool will play a significant role in determining an appropriates pharmacological treatment plan in treating the patient.
