A family health assessment was conducted on the Tumulak family to explore areas of improvement for healthy lifestyles changes. Gordon’s health pattern assessment was utilized, the information collected was able to identify social determinants of health contributing to the family’s health status. SDOH can be utilized to determine appropriate age screenings, determine a health model to assist in creating a plan of action and outlining family centered health promotions.
Social Determinants of Health
The social determinants of health include an individual, relationship, community and society. “The model considers individual fact and how these interplay with factors within the environment and community” (Faulkner, 2018, par. 23). Each factor has the ability to positively or negatively impact the individual’s health. The Tumulak family is economically stable, live in the suburbs of Moorpark, CA. They own a family business and are well known in the Christian community. Each family member has health insurance and available to health services. The Tumulak family has negatively been impacted by personal, health and social relations. Each member of the family struggles with personal situations that interfere with their own growth and acceptance. The daughter’s depression has negatively impacted her mother, Brother David and James. They both struggle and have concerns about how their personal situations may impact their mother. Hassan understanding of his chronic disease creates barriers for optimal health. Jane has full understanding of living with Crohns disease but recent changes in her personal life causes stressors which accidental episodes occur. However, with both the wife and husband working, it makes handling the situation simpler.

Health Promotion
The Tumulak family would benefit from individual screenings. Laila, the mother, would benefit from depression screening as she hasn’t had one in years. James, the father, would benefit from a hypertension screening as well educating the patient on medication compliance. A full understanding of hypertension and regular BP checks. To increase a better outcome in the educating process for Hassan the family and health team should take part in James taking the VARK questionnaire. The VARK questionnaire can help assess the learning styles that would be beneficial. The healthcare team can utilize the transtheoretical Model (TTM) to evaluate James’s readiness to change lifestyle behaviors. Assessment of James’s health literacy can be helpful to adapt changes.
The CDC (2014) describes health literacy as “ a person can obtain, process, and understand basic health information and services that are needed to make suitable health decisions. Health literacy includes the ability to understand instructions on prescription drug bottles, appointment cards, medical education brochures, doctor's directions, and consent forms. It also includes the ability to navigate complex health care systems. Health literacy is not simply the ability to read. It requires a complex group of reading, listening, analytical, and decision-making skills and the ability to apply these skills to health situations”. Better understanding of the causes for Jane’s depression can create a health plan and includes the best treatments. The Tumulak family can be included and supportive towards the treatments that can include psychotherapy, medications, light therapy, exercise and brain stimulation ("Depression | NAMI: National Alliance on Mental Illness," 2017) for minimizing Jane’s stressors to avoid accidental episodes. Understanding the severity of Crohn’s disease is priority for Jane.
According to ("Definitions | Social Determinants of Health | NCHHSTP | CDC," 2014) “patients diagnosed with Crohn’s disease before age 30 years are at greater risk of developing colon cancer. This is because the risk of colon cancer increases for every 10 years a person has Crohn’s disease”. Jane is currently 15 years old and should obtain a colon cancer screening. David should obtain a depression screening as well, due to his stress in medical residency program and the judgments/ disappointment from his family for dating a Hispanic woman. David has expressed his struggle with his parent’s views regarding his decision.
The Tumulak family requires individual health promotions but can also benefit from the family systems theory. Dr. Bowen believed “each individual or subsystem of a family unit or system influences other parts of the system; therefore, an individual cannot be studied in isolation from the family. Dr. Murray Bowen developed the theory from his studies of family problems and processes.” (Whitney, 2018, par.). The Tumulak’s are a very close family that each individual is impacted by individual choices. Understanding the choices of each family member as a whole can create communication outlets for the family as a whole. The goal is for “ individuals create a system in which environment, behaviors, and reactions influence values and morals” (Whitney, 2018). Each family member must be honest and willing to create positive changes.
To conclude, the Tumulak family struggles with the decisions each family member makes. Working together and better communication can create beneficial changes for better health status. Overall the family is concerned and only want the best outcome for each other. Working through models that keep an open line of communication can be beneficial. Better understanding of mental illness health can increase knowledge to the family and be aware of Laila’s depression episodes. The goal is to work together to change negative behaviors to health.
