Broward Health is a community health services provider who works towards improving the health of the communities they work with through the improvement of their health outcomes. The accessibility of the health providers is far reaching since they have six hospitals with outreach services teams in twenty locations. Their services are holistic and cater for all populations in the grass root community level particularly expectant and new mothers. They liaise with faith and community-based organizations in their health services provision. Their community health nursing is primarily aimed at health care information provision, community education on care accessibility and giving referrals for services. They have various programs in place which guide their service provision. The Nurse Family Partnership program caters for prenatal and posts natal healthcare needs; the Healthy Families program caters for expectant and new mothers through home visits while the healthy Start program caters for the screening of mothers and their newborns. They also offer HIV services to the general public as well as a kinship care initiative which ensures that children and their kinship caregivers are catered for in their health care needs.

Identify and describe health-related issues relevant to the population chosen.

The expectant and new time mothers are faced with a lot of challenges concerning their health as well as their babies' health (Abel, 2016). Pregnant women are at a greater risk of developing health issues like unhealthy weight, high blood pressure, diabetes, depression as well as substance use. New mothers are also faced with health issues like postpartum depression; weight-related health problems as well as diet-related health problems which affect both the mother and the baby (Clement, 2012). HIV is a menace that affects the whole population non-discriminatively which makes it a major concentration area for community health nurses. They work towards ensuring that expectant and new mothers have the resources and the know how to take care of themselves as well as for their children (Ezeonwu, 2015). Broward Health has therefore put in place programs to cater for mothers at the prenatal and postnatal levels as well as for creating awareness on HIV to the community.

Broward Health serves the expectant and the new mother’s population. They encounter health related issues during and after their pregnancies which prompt the agency to do home based reach out programs to assess and ensure that the mothers have the required information on the dietary and health requirements needed for both the mother and the child’s wellbeing (Putzer, 2012). Health related issues affecting them include hypertension, obesity, anorexia, pre-eclampsia, diabetes, deep venous thrombosis and anemia (Clement, 2012).Psychosocial welfare for pregnant women and new mothers is crucial in the development and growth of their babies as well as the mother’s health welfare. Depression is a common problem among this population which calls for the information dispensation and creation of awareness role of the community health nursing practice (Abel, 2016). Sound mental and emotional health to avoid consequences of postpartum depression among new mothers is crucial (Pracle, 2015). Community health nurses in Broward Health ensures the psychosocial needs of the population are met through undertaking home visits to new mothers as well as conducting routine apartment complex visits with community-based organizations.

Health promotion is important in improving the health outcomes of any population (Clement, 2012). Health promotion activities encompass targeting individual behavior as well as social and environmental interventions that enhance health outcomes in individuals and the general public. Population health education is an activity that is crucial in creating health awareness and activating lifestyle change for better health (Ezeonwu, 2015). Community health nurses educate pregnant women on the dietary needs and lifestyle choices during and after pregnancy in their antenatal and prenatal care visits. Disease prevention activities also play a crucial role in health promotion. These activities include door to door vaccination programs, healthy living campaigns like hand washing to prevent typhoid and disbursement of preventive measures information to the population (Abel, 2016). Health screening is also a health promotion initiative aimed at testing people to assess their possibility of developing a health problem (Clement, 2012).Community health nursing accomplishes this through setting up screening centers at public places to educate pregnant and new mothers on healthy eating, health habits, and healthy lifestyle choices.

Identify and explain at least two community services for the chosen population.

The Broward Health Nurse-Family Partnership program is a community service that targets pregnant and new mothers (Putzer, 2012). Their child health nurses educate first-time parents on healthy pregnancy practices and healthy parenting and provide their babies with the best possible start in life. Community health nursing ensures that a strong relationship is formed between mothers and nurses which encourage healthy pregnancy and post pregnancy tendencies.

The agency also has a Healthy Families program which entails home visits for expectant and new parents who may be undergoing stressful situations. The visits improve childhood outcomes and empower parents through educating the parents and offering counseling services to improve the families' self-sufficiency (Putzer, 2012). The program also provides for infant screening and assigns every new mother with a registered nurse who follows up the mother and child progress till the child turns two years old.

Identify and describe potential environmental hazards for the chosen population.

Potential environmental hazards for expectant and new mothers include cleaning products like oven cleaners which contain glycol ether which is a miscarriage enabler and could lead to the development of birth defects in the unborn child or even fetal death. Exposure to radiation from mobile phones can also have adverse effects on both the mother and the baby. It can cause retardation, deformity, cancer and congenital disabilities. Water is also a potential environmental hazard to expectant and new mothers (Putzer, 2012). Lead in drinking water can cause miscarriage and deformity in the baby.
