Martin Luther King Jr, Community Hospital is a private, non-profit healthcare organization located in Los Angeles. The hospital provide general acute care, general medicine and surgery, neurology, nephrology, obstetrics, and other medical services (MLK Community Health Foundation, 2017). The hospital uses data-driven care model to care for its patients. The choice of a data-driven care model was driven by several factors such as the lack of sufficient infrastructure and a corporate superstructure. Additionally, the hospital faced a shortage of primary caregivers and clinicians in the underserved community the hospital assisted (MLK Community Health Foundation, 2017). The hospital had to adopt a data-driven care model to ensure that it supports the patients while at hospital and in the community. Considering that the facility is a131-bed hospital, adopting a care model that makes patients receive the same level of care like larger hospitals was necessary. 

A data-driven care model has several benefits. First, it has the potential to enhance inter-professional collaboration. Since a data-driven care model uses real-time analytics, it facilitates the tracking and updating of patient data throughout the care journey. It enhances inter-professional collaboration because it ensures that all care providers are at the same level all the time from entry to discharge. The model binds all care teams together through a realistic dashboard. A data-driven model can contribute to the delivery of cost-effective healthcare because it promotes patient-centered care. The model enables providers to prioritize patients using their risk-scores or schedule (Madsen, 2014) . Through the model, care managers can generate work queues based on demographic factors, availability of provider and patient, and availability of conveyance. Additionally, it helps create customizable care plans that comprise automated outreach mechanism and transportation through a single click of a button (Shashank, 2018). Thus, the model minimizes the complexity of demanding tasks, which in turn lowers the cost of healthcare.
